RESOLUTION NO. 2017-399
RESOLUTION APPROVING RENEWAL AGREEMENT WITH
WELLMARK BLUE CROSS BLUE SHIELD FOR FEES ASSOCIATED
WITH ADMINISTRATION, NETWORK ACCESS AND STOP LOSS
AND AUTHORIZE MAYOR AND CITY CLERK TO EXECUTE SAID
DOCUMENTS.
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the contract dated May 22, 2017, for the renewal of administration, network access, and stop
loss health insurance with Wellmark Blue Cross Blue Shield, for an estimated premium of
$1,309,134, for the period of July 1, 2017 through June 30, 2018, is hereby approved and the
Mayor is authorized and directed to execute the same on behalf of the City of Waterloo, Iowa.
PASSED AND ADOPTED this 22nd day of May 2017.
