You are hereby ordered to make the following changes from the plans and specifications or perform the
following extra work on your contract dated April 21, 2014.
A. Description of change to be made or extra work to be done:
4001 Installing new sanitary sewer along Fisher Drive
4002 Installing new sanitary manholes along Fisher Drive
4003 Traffic control during construction of new sanitary sewer on Fisher Drive
B. Reason for ordering change or extra work:
4001-4003 Installation of new sanitary sewer along Fisher Drive prior to the Green Acres Storage
building being completed. By installing the sanitary sewer prior to the Green Acres Storage
development there will not be any costs for replacement of landscaping, driveways or repair of the
detention basin.
C. Settlement for cost of work to be made as follows:
4001 398.00 L.F. x $60.50/L.F. = $24,079.00
4002 2 Each x $4,785.00/Each = $9,570.00
4003 1 L.S. x $500.00 = $500.00

Background Information:
Installation of new sanitary sewer along Fisher Drive prior to the
