Summary Statement:
There has been additional work on this project for the construction
review services including reviewing the qualifications of non -DOT
preapproved suppliers as other preapproved suppliers refused to do the
work based on the unusual graphics on the signs. There were additional
services provided to address working day issues.
Finalization and project closeout work remains on this project and this
supplemental agreement covers the anticipated hours for remaining work
and estimated maximum not -to -exceed cost of $5,510.00.

SUPPLEMENTAL AGREEMENT #2
WHEREAS, a Professional Services Agreement is currently in place between the City of Waterloo, Iowa ("City") and
JDE Engineering, PLC ("Consultant") dated March 21, 2011, and
WHEREAS, additional Construction Related Services were determined to be necessary to assist in obtaining qualified
sub -contractors to work on the project, and to also document quality of construction due to an increase in the
duration of the project schedule,
NOW THEREFORE, it is mutually agreed that this Supplemental Agreement #2 shall consist of the following:
SCOPE OF SERVICES
The scope of services shall consist of obtaining the following:
• Work with the prime contractor and Iowa DOT to obtain a qualified sign manufacturing sub -contractor,
pole manufacturing sub -contractor, and finish painting sub -contractor to complete work on this project,
• Conduct research into the supporting documentation to justify additional working days worked by the
contractor to complete the project,
• Meet with contractor, Iowa DOT staff, and City personnel to discuss possible resolutions to excess
working day issues,
• Work with the Iowa DOT to resolve excess working days issues documented during the course of the
construction in a satisfactory manner in accordance with Iowa DOT and Federal Highway Administration
(FHWA) guidelines.
II. TIME OF BEGINNING AND COMPLETION
Work under this supplemental agreement is complete as of the date of this agreement.

III. COMPENSATION
The fee for the work detailed under "Section I. Scope of Services" is based document time -and -materials
worked by JDE Engineering staff (see attachment). The total cost of Supplemental Agreement #2 is
$5,510.00. This fee is considered a maximum not -to -exceed cost.
