Recommended City Council Action:
Staff has reviewed this Change Order and recommends that the Mayor be authorized to sign it on
behalf of the City.
Summary Statement
Change Order No. 1 is for the increased amount of $22,069.42. The work related to this change
order is due to field conditions that were unknown at the time the project was designed.

You are hereby ordered to make the following changes from the plans and specifications or perform the
following extra work on your contract dated April 21, 2014
A. Description of change to be made or extra work to be done:
1001 Remove existing paving — widening section and patch
1002 Replace pavement displacement with Modified Subbase
1003 Remove existing driveway or paved shoulder
1004 Replace driveway or paved shoulder, 6" PCC
1005 Replace miscellaneous paving at Leversee Rd.
1006 Trim the subbase
1007 Jackhammer edge of existing paving at Leversee Rd.
1008 Place sand in spalled areas of old paving
1009 Delete fabric under full width paving where subbase was placed
B. Reason for ordering change or extra work:
1001 Widening of 3'x10" paving section was too high
1002 Bring area to grade for paving
1003 Removal of existing driveway or paved shoulders was not included in original plans
1004 Replace existing paved driveways or shoulders to adjoin new paving
1005 Additional paving at intersection needs replacement to tie into new — handwork
1006 Get the subbase area ready for paving — not included in original plan
1007 Additional removal at BOP due to existing paving section — special removal method
1008 Need to fill in low areas on old concrete to provide a uniform surface for overlay
1009 Portion of paving placed on subbase did not need fabric that was specified for overlay
