Recommended City Council Action: Attached for your consideration and approval is a copy
of Change Order No. 2 for LaPorte Road Pumping Station and Force Main. Change Order
No. 2 includes 7 additional changes : Seeding/fertilizing/mulching for hydraulic seeding,
explosion proof electrical boxes, additional float, alarm equipment, additional conduit, water
service, wood excelsior mat.

Background Information: This project includes the construction of a new lift station,
approximately 1,880 L.F. of gravity main, approximately 2,960 L.F. of force main, one air
release manhole, seven sanitary sewer manholes and incidentals located along LaPorte Road.
Work also includes removal of the existing lift station, installation of a manhole once the
new statin is operational, and grading and surface restoration.

22 Seeding, Fertilizing and Mulching for Hydraulic Seeding (0.6 AC @ $2,690.00)

2-2 Aciffitional Float in Wet Weil
2-3 Alarm Equipment at Pump Station
2-4 Additional Conduit from lighting Panel to Meter Pit
2-5 Water Service for Pump Station
2-6 Wood Excelsior Mat

Reason for Change Order
22 & 2-6 During SWPPP Inspections by City staff it was noted that there were some erosion Issues In the newly seeded
areas of this project. In order to take care of these issues it was requested that the areas in question be
re -graded, re -seeded, and covered with wood excelsior mat. The existing wattles will also need to be
removed and replaced. This item covers the purchase and installation of the matting, the grading and seeding
work, and the removal and reinstallation of the existing wattles.
2-1 During an initial electrical inspection, it was requested that three junction boxes be changed to explosion proof boxes
due to their Class 1, Division 1 location. This item Is for the removal of the original boxes and the purchase and
installation of the new boxes.
2-2 During pump testing the City WPCF staff requested that an additional float be added to the wet well to allow easier
maintenance of the pumps. This item rovers the purchase, installation and wiring of this float
2-3 The City WPCF staff requested that the contractor provide and install the alarm equipment for the pump station instead
of the City as shown In the contract documents. This item covers the purchase and Installation of this equipment_
2-4 The City WPCF staff requested additional conduit be placed from the lighting panel to the wet well for future use. Thi<
item covers the purchase and installation of this conduit.
2-5 An existing water service was found to be plumbed to the original pump station at E Orange Road. WPCF staff requested
that this service be extended to the new pump station and a yard hydrant installed for use during maintenance. This item
covers the purchase and Installation of this water service and hydrant.
