The City of Waterloo hereby agrees to extend the agreement for office supplies, toner and
copy paper, in accordance with the terms and provisions of the original agreement on
October 24, 2011, with Iowa Office Interiors Inc. dba Matt Parrott/Storey Kenworthy of
Waterloo, Iowa, upon the following terms and conditions:
1. Iowa Office Interiors Inc. dba Matt Parrott/Storey Kenworthy shall provide office
supplies/toner/paper to each department for the City of Waterloo for the period of
July 1, 2013 -June 30, 2014; prices are subject to change quarterly. A 30 day
notice will be given to the City of Waterloo when prices increase. There will be
no delivery charges.
2. Iowa Office Interiors Inc. dba Matt Parrott/Storey Kenworthy shall provide each
individual department of the City of Waterloo separate billings for office
supplies/toner and copy paper, as well as new catalogs every year.
3. The prices designated in the original bid from Iowa Office Interiors Inc. dba Matt
Parrott/Storey Kenworthy shall be the prices charged to the City of Waterloo for
office supplies/toner and paper for the period of July 1, 2013 -June 30, 2014.
4. This agreement incorporates by reference the RFP as part of the contract.
5. The City of Waterloo reserves the right to cancel this agreement at any time by
giving Iowa Office Interiors Inc. dba Matt Parrott/Storey Kenworthy a thirty (30)
day advance written notice.
