Recommended City Council Action: Approval of supplemental Agreement No. 1 for
construction related services for the La Porte Road Pump Station and Force Main project
and amend the original Professional Service Agreement dated October 15, 2012.
Summary Statement: A Professional Service Agreement was entered into by the City of
Waterloo and AECOM Technical Services Inc. dated October 15, 2012 for design and
bidding services for a new (replacement) pumping station and new force main to serve
the existing Southview Estates and Cedar Hills developments and future development
along La Porte Road in South Waterloo.

LAPORTE ROAD PUMPING STATION AND FORCE MAIN
CONSTRUCTION -RELATED SERVICES
SUPPLEMENTAL AGREEMENT NO. 1
WHEREAS, a Professional Service Agreement was entered into by the City of Waterloo (Client), 715
Mulberry Street, Waterloo, Iowa, and AECOM Technical Services, Inc. (ATS), 501 Sycamore Street,
Suite 222, Waterloo, Iowa, dated October 15, 2012, for design and bidding services for a new
(replacement) pumping station and new force main to serve the existing Southview Estates and Cedar
Hills developments and future development along LaPorte Road in south Waterloo; and
WHEREAS, the Client and ATS now desire to enter into Supplemental Agreement No. 1 for
construction -related services for the project.
NOW THEREFORE, it is mutually agreed to amend the original Professional Service Agreement as
follows:
I. PROJECT DESCRIPTION
The project includes the construction of a new lift station, approximately 1,880 L.F. of gravity
main, approximately 2,960 L.F. of force main, one (1) air release manhole, seven (7) sanitary
sewer manholes and incidentals located along LaPorte Road. Work also includes removal of
the existing lift station, installation of a manhole once the new station is operational, and grading
and surface restoration.
II. SCOPE OF SERVICES
The Scope of Services is for construction -related services and includes the following:
1. Assign a project manager who will coordinate project activities and will be the principal
liaison between the Client, Consultant and regulatory agencies.
2. Assist the Client in assembling the contract documents for execution by the Contractor
and the Client.
3. Conduct a preconstruction conference with representatives of the Contractor, Client,
Consultant and affected utilities.
4. Provide horizontal and vertical control for the project including:
A. Grade Stakes — Alignment and grade for sanitary sewer and manholes.
B. Construction Limits - Identify where needed. Property staking or surveys are
not included within the scope of this item.
5. Review shop drawings and other submittals as required of the Contractor by the
contract documents for conformance with the design concept of the Project and
compliance with the information given in the contract documents.
6. Answer design interpretation questions from the Client, Contractor, review staff and
appropriate agencies.

7. Process monthly applications for payment submitted by the Contractor and forward to
the Client for execution with recommendations for approval and payment.
8. Witness/provide, where applicable, field testing of the facilities furnished under the
contract to assist in determining conformance with the contract documents.
9. Visit the construction site by design personnel at appropriate stages of construction to
review the quality of the work and to determine, in general, whether the work conforms
with the contract documents.
10. Assist the Client and Contractor in preparing and processing contract change orders.
11. Provide periodic field observation during construction to review the work of the
Contractor to determine if the work is proceeding in general accordance with the
contract documents and that completed work appears to conform to the contract
documents. Staffing requirements may be adjusted during the project in relation to the
level of construction activity.
12. Report to the Client any work believed to be unsatisfactory, faulty or defective or does
not conform to the contract documents, and advise the Client of any work that should
be corrected or rejected.
13. Consider and evaluate Contractor's suggestions for modifications and report them with
recommendations to the Client.
14. Maintain files for correspondence, reports of the job conferences, shop drawings and
sample submissions, reproductions of original contract documents including addenda,
change orders, field modifications, additional drawings issued subsequent to the
execution of the contract, Engineer clarifications and interpretations of the contract
documents, progress reports and other project -related documents.
15. Keep a diary, log book or report for those times on site, recording hours on the job site;
weather conditions; data relative to questions of extras or deductions; list of visiting
officials and representatives of manufacturers, fabricators, suppliers and distributors;
activities; decisions, observations in general and specific observations in more detail
when necessary, as in the case of observing test procedures.
16. Conduct a review of the project near completion and prepare a list of items to be
completed or corrected.
17. Perform a field observation of the completed project before a final application for
payment is processed for the Contractor.
18. Provide the Client with a copy of revised drawings of the construction plans for the
project based on the construction observation records of the review staff and
Contractor showing those changes made during construction considered significant.
19. Assist the Client with the final close-out documentation from the Contractor.

III. COMPENSATION

Compensation for the Services shall be on an hourly basis in accordance with the hourly fees
and other direct expenses in effect at the time the services are performed. Total compensation
is an estimated fee of Sixty -Five Thousand Five Hundred Dollars ($65,500.00) and shall be
segregated from the fees estimated for Part VI of the original agreement.
IV. In all other aspects, the obligations of the Client and ATS shall remain as specified in the
Professional Services Agreement dated October 15, 2012.
IN WITNESS WHEREOF, the parties hereto have executed this Supplemental Agreement No. 1 as of
the dates shown below:
