EXTENSION OF GERALDINE ROAD
UTILIZING REVITALIZE IOWA SOUND ECONOMY
(RISE) PROJECT FUNDING
SUPPLEMENTAL AGREEMENT #1
WHEREAS, a Professional Services Agreement is currently in place between the City of Waterloo, Iowa ("City")
and JDE Engineering, PLC ("Consultant") dated December 12, 2011, and
WHEREAS, additional property boundary survey information was determined to be necessary to develop the
final design of the proposed roadway extension,
NOW THEREFORE, it is mutually agreed that this Supplemental Agreement #1 shall consist of the following:
I. SCOPE OF SERVICES
The scope of services shall consist of obtaining the following survey:
• Legal boundary survey of parcel #8913-08-201-004 and 8913-08-201-005, 3261 W. Airline Highway,
Waterloo, Iowa, Owner: Glenn Behrends. Survey includes obtaining existing property corner
information and southerly and westerly existing building locations.
• Additional survey information will be incorporated into the design of the proposed extension of
Geraldine Road. Geraldine Road will be extended approximately 900 feet to the west.
II. TIME OF BEGINNING AND COMPLETION
Work under this supplemental agreement shall begin following approval of the City Council and
execution of this agreement by the Mayor of the City of Waterloo. Work shall be completed within 2
weeks from the date of approval of this Supplemental Agreement.

III. COMPENSATION
The fee for the work detailed under "Section I. Scope of Services" is based on a lump sum cost to
complete the work. The total cost of Supplemental Agreement #1 is $1,900. This fee is considered a
maximum not -to -exceed cost.
IN WITNESS WHEREOF, the parties hereto have executed this Supplemental Agreement #1 as of the dates
shown below:
