Recommended City Council Action:
Approve recommendation of award of change order #1 in the amount of $67,668.84 to
Americlean of Waterloo, Iowa in conjunction with Five Sullivan Brothers Convention
Center Duct And Coil Cleaning, be received, placed on file and adopted.
Summary Statement Addition work to be done by Americlean will help protect the
Five Sullivan Brothers Convention Center heating and cooling system from future
problem. This type of work was not part of the original contat but is very important for
future use of the Five Sullivan Brothers Convention Center.

This RFC is the 2nd in a series to be submitted as information and recommendations are best determined. As before,
this segmented approach is deemed best to allow for the project to continue, utilizing industry recommended protocols
for addressing mold contaminates, and to assist in restoring the contaminated lining materials within the HVAC
systems.
These recommendations address the application of EPA registered sanitizing/disinfecting agents and
antimicrobial/fungicidal HVAC coatings, each registered specifically for use in HVAC Systems.
HVAC Systems #3, 4 and 5.
Recommendation
Per Project IEP recommendations, included the HVAC and Environmental Assessment (performed by Joel Koffron,
Environmental Engineer -IEP). Contractor to provide the following services:
1) Supply OSHA required PPE to be donned by workers during application of sanitizing/disinfecting agents and
antimicrobial/fungicidal coating materials to the interior of the contaminated ductwork.
2) Apply Oxine- EPA registered HVAC sanitizing agent to all supply (HP and LP) and return ductwork on HVAC
systems #3, 4, and 5.
3) Apply Surface Shield- EPA registered antimicrobial HVAC encapsulate to all Low Pressure supply ductwork
and all return ductwork on HVAC systems #3, 4, and 5. Clear coating- ULV application.
4) Apply IAQ 8500 or Fosters 40-30- EPA registered antimicrobial/fungicidal HVAC coating to all HP supply
ductwork on HVAC systems #3, 4, and 5, from the AHU to the VAV's. Black coating- airless sprayer/ lance
application.
5) Provide protective services- drops, containments, visqueen as required for applications.
6) Furnish and install additional access doors as required for application of products as above
7) Supply lift equipment for application of sanitizing and coating agents.
*Removal and replacement of degraded or unsalvageable interior lining materials in the AHU's and any HP supply
ductwork is not included, and will be addressed on another RFC as information and recommendations are available.

ENVIRONMENTAL SERVICE AGREEMENT
This AGREEMENT is by and between:

1) Allow Americlean of Iowa LLC to provide environmental consulting services which will include:
(a) Performing a visual inspection of all ductwork for each of the currently installed and
functioning air handling mechanical units to include AHU numbers 3, 4 and 5.
(b) Perform a minimum of two (2) surface samples from each of the supply side unit ducting
insulation linings and a minimum of one (1) per return main duct runs. This would be a total of
nine (9) surface samples.
(c) Perform air sampling of a minimum of two (2) ambient air sample in one of the main areas in
which each of these areas service, minimum of six (6) indoor air samples with one (1) outdoor
baseline collected for a total of seven (7) air samples.
(d) Collect all data and work with the owners selection of mechanical contractor to make
suggestions to the owner as to the direction and scope of work that would be felt necessary to
provide a Condition 1 Environment within the building and to restore the mechanical systems (as
defined in the applicable standards noted) therein.
(e) We recommend conducting a complete Post Remediation Verification (PRV) sampling at the
completion of all work to verify that a Condition 1 Environment exists within the building in the
areas inspected. This is not included and cost/scope to be determined.
2.) Fee Schedules for Services
Environmental Engineer / IEP - $98.50/hr (site and documentation time)
Technician for Assessment - $68.50/hr (assist IEP, provide access to ductwork)
Samples - $99.50/ea (this includes shipping, restock costs and laboratory fees)
Anticipated time for IEP time — 18 hours = $1,773.00
Anticipated time for Technician - 9 hours = $616.50
Sampling Fees —16 samples — $1592.00
Total fees — Not to Exceed - $3981.50
Terms: As outlined on the original contract for this project.
If further work is warranted on your project based on any recommendations by Americlean of
Iowa LLC, other professionals, requested by the Client, or if unforeseen conditions arise, then
Americlean of Iowa LLC will notify the Client of the change in scope of the project and charge
Time, Material, and Subcontract rates in accordance with the Americlean of Iowa LLC costs and
all other fees and costs which are due pursuant to this service agreement.
3.) The industry standard remediation provisions set forth in the IICRC S520 & ACR 2006 general
conditions and guidelines and are incorporated hereunto and made a part of this agreement.

This RFC is the first in a series to be submitted as information and recommendations are best determined. This
segmented approach is deemed best to allow for the project to continue, utilizing industry recommended protocols for
addressing mold contaminates, and to assist in reducing impact to workers and building occupants.
The intent of these recommendations are to provide for OSHA required PPE, Site Protection (Cleaning of ambient air
during cleaning processes), and containment/isolation strategies as required during the remaining cleaning efforts of
HVAC Systems #3 and 4.
Recommendation
Per Project IEP recommendations, included the HVAC and Environmental Assessment (performed by Joel Koffron,
Environmental Engineer -IEP). Contractor to provide the following items during the remaining cleaning of HVAC
Systems #3 and #4:
1) Supply OSHA required PPE to be donned by workers during cleaning of the contaminated ductwork
2) Furnish HEPA filtered AFD's (Air Filtration Devices) in reasonable, sufficient quantity and sizing in work areas of
facility to clean/scrub ambient air to reduce airborne contaminates
3) Implement localized containment cubes or barriers as required in areas that do not compromise worker or occupant
safety to isolate ductwork openings/work areas during cleaning efforts
4) Furnish HEPA AFD's in each work area in close proximity to the cleaning efforts to control and capture airbome
contaminates during cleaning efforts
5) HEPA AFD's quotation to include set-up, tear down, monitoring as required, decontamination, and filter changes as
required
