Note- During meeting for determining Scope of work changes, AHU #4 was specified to
be deleted, however AHU #4 services the ductwork located on Upper floor, so to be
included
All work to be performed according to requirements set forth in bid request for Five
Sullivan Brothers Convention Center Duct and Coil Cleaning.
All above work performed with compliance to: NADCA Standards 1997-05, 1992-01,
ACR 2006, ASHRAI Standard 62-1989, NFPA Standard 90-A, SMACNA guidelines,
NAIMA standards, EPA & OSHA Standards, IESO Standards.
Payment terms are as follows: Billing will be sent in three (3) invoices, in amounts of one
third of total contract price, each, on June 1, 2011, July 1, 2011 and August 1, 2011.
Payment terms are net thirty (30) days from invoice date. Payments not received within
thirty (30) days will be charged interest at a rate of 18% (equal to 1.5% per month).
Project completion date is August 1, 2011. Extensions to this date will result from delays
in working on project, due to events scheduled at the Five Sullivan Brothers Convention
Center or delays requested by or caused by the City of Waterloo, without penalty to
Americlean.

The above prices, specifications and conditions are satisfactory and are hereby accepted.
