Waterloo Fire Rescue would like to thank the Black Hawk County Gaming Association in advance for your time and consideration in this grant review process. Waterloo Fire Rescue here after known for the purpose of this application as (WFR) provides fire, rescue, emergency medical services, fire inspections, public education and community services for the citizens of Waterloo. WFR also has contracts with Elk Run Heights and East Waterloo Township's to provide fire, rescue and emergency medical services encompassing approximately eighty square miles. WFR provides a hazardous materials response team that is responsible for haz-mat services for ten counties located from the Iowa/Minnesota boarder to Interstate 80 and also staffs a technical rescue team to provide confined space rescue and high/low angle rope rescue activities. The nature and condition of work performed by fire fighters pose serious hazards to their health and life. The main problems arise from exposure to multiple physical (physical and thermal workload) and chemical (toxic chemicals) agents, as well as from high levels of stress experienced during emergency operations. The health impairments and disorders determined by the job characteristics include injuries, traumas, respiratory disease, cardiovascular diseases, lung and other cancers. In recent years the workload of fire fighters has increased considerably due to the expanded scope of our duties. The two areas of concern that we are trying to address with this grant funding are musculoskeletal injuries and heart and lung related disabilities. Bone and muscle injuries although not always career ending are extremely costly to the City of Waterloo in workman's comp and healthcare costs as well as the added cost to the department to cover short staffing in the event of long term injuries. This has a negative impact in the department's budget and ability to adequately respond with the appropriate number of personnel. Heart and lung disabilities are career ending and there are Tong -term expenses that the city incurs for the duration of that employees life span. In January and April of 2010, twenty WFR members participated in structured physical fitness program. The University of Northern Iowa assisted in this study measuring body fat index as well as the body's ability to perform for long periods of time in a highly physical demanding environment. The results were incredible as you can see in attachment from Dr Lund of the University of northern Iowa. In order to move forward with this program WFR is requesting funding for identical fitness equipment to place in all six fire stations. This would allow fire fighters to exercise at work as well as use the equipment during their off duty hours. WFR will make this equipment available for use by all 500 plus city employees making this a more comprehensive grant request. WFR has on its staff five athletic trainers that will also be designing and monitoring fitness programs for WFR personnel. In order to minimize the grant request WFR has developed partnerships with local businesses to provide in-kind donations that greatly reduce the amount of funding needed. WFR is submitting two plans for your review and consideration. The first plan (A) includes all necessary equipment needed to accomplish our goals. The total cost of all equipment needed is $70,776.59 with a funding request after in-kind donations of - If this amount is in excess of allowable funding we have a plan (B) that includes limited musculoskeletal equipment but puts a heavier emphasis on cardiovascular equipment. The total cost of equipment needed is $49,704.89 with a funding request after in-kind donations of . Both plans include commercial grade treadmills, rowing machines and free weights. WFR will also provide the necessary labor to build lifting platforms at each fire station. Working with partnerships such as the Black Hawk County Gaming Association and the local businesses that are providing the in-kind donations we will be better prepared to accomplish WFR's Mission statement: "To Protect the Lives and Property of the Citizens of Waterloo" and "To Create a Safety Conscious Community". If awarded WFR will take delivery, perform all necessary set-up, construct lifting platforms and have fully functional exercise areas at all stations with -in 90 days. The request includes commercial grade equipment that should increase the life spam of items purchased and we have included warranty information on the cardio equipment requested. Through education and physical fitness WFR plans to reduce heart and lung related disabilities and reduce long-term musculoskeletal injuries that burden the City of Waterloo and ultimately the taxpayers themselves. (319).273-3615 robin.lund@uni.edu Mr. Graham, This report is in response to a request for research assistance from the Waterloo Fire Rescue (WFR). The purpose of the original request was to assist the WFR in ascertaining the pre and post physical fitness and body composition of firefighters participating in the 12 -weeks of CrossFittraining at United Sport and Athlete Inc., in Waterloo, IA. The remainder of the report will be divided into three sections. These include (1) methods and procedures of the study, (2) results of the study and (3) practical implications of the study. . Methods and Procedures Subjects: Nine males from the WiRF, volunteered to be assessed for the study. Descriptive data (Mean ± SD) include age (36.8 ± 4.4 years), height (70.4 ± 2.8 inches) and weight (227.7 ± 53.6 lbs) upon arrival to the laboratory for pretesting. Equipment: Metabolic measurements (maximal rate of oxygen consumed, V©z,,,a,„ ml/kg/min) were obtained using a Parvo Medics True One 2400 Metabolic Measurement System (Sandy, UT). Heart rates were obtained using Polar Accurex ii monitors with chest transmitter (Polar Electro, Finland). A Monad( Ergomedic 828 E cycle ergometer (Stockholm, Sweden) was used for the cycling bouts. Power output was calculated via custom software. Vital capacity was estimated with an 8 L capacity wet spirometer. Underwater weights were obtained using the Flotaweight underwater weighing system (Exertech, Dresbach, MN). Testing Procedure: Upon arriving the subjects were asked to provide their name and age. Height and weight were measured using standard laboratory procedures. Vital capacity and underwater weighing were used to measure BF%. After the underwater weighing, subjects performed a 5 -minute warm-up of cycling at a comfortable pace (10-20 rpm) with no resistance on the cycle ergometer. After the first minute, subjects performed a set of three sprint interval (5 seconds In duration) with 10 - seconds of rest in between Intervals. A final sprint interval with the prescribed load (7% of body mass) was performed. The subjects pedaled for 30 -seconds with no resistance (10-20 rpm). At this point, the subject was asked to pedal as fast as possible while the researcher communicated a 5 -second countdown as prompted by the Wingate software. Upon the audio prompt from the software, the researcher loaded the cycle ergometer with the prescribed Toad. The subject was encouraged to pedal as fast as possible for a 15 -second interval. Upon completion, relative peak power (PPrei, Watts/pound, W/ib) was recorded. After the cycling test, subjects were fitted with the headgear and mouth piece used for metabolic testing. A treadmill protocol was designed to assess their aerobic physical fitness via the VO2m. The protocol consisted of a warm-up phase and a VO2m.c phase. The warm-up consisted of 3 -min. at 3.0 mph. The VO2matt phase started at 5.0 mph. The speed of the treadmill was increased by 0.5 mph each minute until exhaustion. The maximum rate of oxygen consumption (VO2mam ml/kg/min) was recorded. Variables of interest: -Body fat percentage (BF%): The proportion of the subject's body mass that is made of fat. -Relative peak power.(PPrei): The maximum rate at which work was performed per unit of body weight during the 15 -second cycling test. The relative value was reported due to the reduction in body weight that occurred during the study. -Aerobic fitness (VO, ): The maximum rate at which oxygen is consumed, relative to body weight (ml/kg/min). Statistical Analysis: Descriptive statistics were calculated for all variables. Paired samples t-tests were calculated to determine whether or not the improvement observed after the 12 -week test could be considered "statistically significant." A statistically significant improvement means that the observed improvements are most likely due to the 12 - week CrossFit training regimen as opposed to chance. Purpose of the study: The purpose of this study was to examine the effects of a 12 -week CrossFit training regimen on physical fitness and body composition in firefighters. Results Descriptive statistics of the aforementioned variables are provided in Table 1 for both the pre and post. The results of the statistics indicated that the improvements in all of the variables were "statistically significant" Figures 14 display the improvements graphically. Figure 1. SS81111131qa1Ge Uaural) 'E aualA Practical Implications Previous work has determined that physical fitness and body composition play a role in the ability of firefighters to perform firefighting tasks (DM Sal, et al., 2009). Although firefighters have ample time to exercise regularly, many do not maintain even the most basic standards for fitness. Donovan et al. (2009) reported that 25% of their sample did not meet the accepted minimal standard of aerobic fitness (42 mI/icg/min). Furthermore, 15% of the sample met the NCEPJATP 111 diagnostic criteria for the metabolic syndrome. Crossfit Is a relatively new affordable training program that is especially popular with police academies, tactical operations teams, military special operations units and martial artists. Its popularity seems to be based on the broad, general and inclusive approach to training, designed to improve body composition, power and conditioning (anaerobic and aerobic fitness) which are required by the aforementioned populations. On average, the fitness of the subjects was below the reported recommended levels (^35 ml/kg/min) based on the demands of the job when they started the program. Considering that the leading cause of death in firefighters (on the job) is cardiac arrest, the timing of this study could not be better. From a physiological standpoint, because of the aforementioned fitness levels; this group was highly trainable and the potential for improvements was very good. Clearly the program was able to improve thephysical fitness, body composition and the work capacity of the subjects. More specifically, there was a 5% improvement in body weight, a 24% improvement in aerobic fitness, a 32% improvement in BF%, and a 9% improvement in relative peak power in the cycling test. There is evidence to suggest that these subjects would continue to make improvements as their training progresses. Three WFR employees who have engaged in CrossFit (five days/week) for over 12 months also agreed to participate in this study during the pretest phase. At the time of testing these subjects had superior numbers in BF% (13.1%), VO2,r, (53.2 ml/kg/min) and relative peak power (5.3 W/Ib) when compared to the experimental group. Although it may not be reasonable to expect all WFR employees to commit to this level of training, it would be reasonable to expect a 3 day/week commitment which would allow for the maintenance or slight improvement of current levels. Although blood was not drawn for this study, it can be assumed that the improvements in body composition and aerobic fitness will reduce the risk of heart disease and diabetes in these subjects. Currently there appears to be a lack of consensus in the literature on the ability of such programs to reduce health care costs (Aldana, 2001; Shepard, 1992). Most likely, the unique circumstances of each individual situation will determine the cost to benefit ratio. Considering the relatively low cost of such a program, it is recommended that the city Investigate how the implementation of this type of program could affect the cost of insuring WFR employees in order to get a clear picture of how this may impact the city financially. In conclusion, 12 -weeks of CrossFit training significantly improved body composition, work capacity and aerobic fitness in the WFR employees. It is recommended that these subjects continue to engage in regular vigorous physical activity at least 3 days/week. References Aldana, S. (2001).
