ASBESTOS ABATEMENT SERVICES
This First Amendment to Contract for Asbestos Abatement Services is made as of the
latest date set forth below by and between the City of Waterloo, Iowa ("City") and Advanced
Environmental Testing and Abatement, Inc. ("Contractor").
WITNESSETH:
WHEREAS, the City and the Contractor have heretofore entered into a certain Contract
for Asbestos Abatement Services dated February 15, 2011 (the "Agreement") for the Contractor
to provide asbestos abatement services to the City pertaining to the City of Waterloo's Hazard
Mitigation Grant Program (HMGP) Property Acquisition Project; and,
WHEREAS, pursuant to the Agreement, the Agreement may not be modified or
amended except pursuant to the mutual written agreement of the parties; and,
WHEREAS, the Contractor has determined that there is additional asbestos containing
material (ACM) that was not identified by the asbestos testing and survey reports, and that the
additional ACM must also be abated, and that the cost to abate the additional ACM is not part of
the scope of work covered by the Agreement; and,
WHEREAS, the Contractor has provided a bid tab for Change Order No. 1, with a net
increase of $880 for work performed under the Agreement.
NOW, THEREFORE, the parties hereby amend the Agreement to approve Change
Order No. 1 with a ne. _t increase of $880 for work performed under the Agreement, for a revised
contract limit and total actual expenses allowed by the Agreement not to exceed $87,590. All
other terms and conditions of the Agreement shall remain in full force and effect, without
modification.
IN WITNESS WHEREOF, the parties have caused this First Amendment to Contract
for Asbestos Abatement Services to be executed by their duly authorized representatives.
