The undersigned parties hereby agree to modify the above referenced agreement
according to the terms and conditions described below:
1. Work & Items
The following items required extra work that was not anticipated at the time of signing
the agreement:
• Design changes based on Iowa DOT's review of preliminary plans. Mast arm
pole configuration was changed to keep the existing truss.
• Additional surveying and design effort was also necessary when it was
determined that the existing roadway culvert to drain the median adjacent to the
lengthened northeast -bound US 63 left -turn lane could not be utilized and that
two new pipe culverts would need to be jacked under the southwest -bound lanes
ofUS63.
• Changes to plans and specifications format to accommodate a DOT letting.
2. Costs:
The Agreement amount is increased by $6,500. The original agreement amount was
$32,900. The new "not to exceed" amount is $39,400.
