You are hereby ordered to make the following changes from the plans and specifications or perform
the following extra work on your contract dated April 12, 2010.

Description of change to be made or extra work to be done:
1001 Remove existing asphalt trail and replace with 5 -inch PCC
1002 1-1 /2 -inch roadstone backfill
1003 Additional traffic control for washout repair area
1004 Asphalt trail patching — 3 locations
1005 Additional removal of existing asphalt trail and replacement with 5 -inch PCC
Reason for ordering change or extra work:
1001 90 -foot section of existing asphalt trail damaged during Spring 2010 flooding
1002 Damaged trail section was undermined, existing material washed away
1003 Additional traffic control around washout repair area
1004 Damage from tree roots and maintenance vehicles that possess safety problems to users.
Areas acceptable during fall Inspection, but conditions deteriorated over the winter.
1005 Received an excellent bid from contractor and further review of funds showed monies available
to make additional trail repairs beyond that amount originally put out for bid.
Settlement for cost of work to be made as follows:
1001 Repair trail section damaged by flooding
1002 1 -1/2 -inch roadstone backfill
1003 Additional traffic control for washout area
1004 Asphalt trail patching
1005 Additional replacement of asphalt trail
