Recommend for approval by the City Engineer.
Recommended Action: This waiver is needed due to the placement of a driveway or sidewalk on City right-of-way on an
unimproved street.
I have reviewed this request and recommend its approval subject to the following provisions.
1.Work to be performed by an approved and bonded contractor.
2.A permit is to be obtained from the office of the City Engineer prior to construction.
3.All work shall be performed under the supervision of the City Engineer and at no cost to the City of
Summary Statement: Waterloo.
$7.00 cash for the purpose of recording this waiver and a copy of the legal description have been
provided to the City Clerk's office.

special surface texture to be used on the concrete approach (i.e., exposed aggregate, brick
stamped pattern, paving brick).
elimination of the sidewalk section due to the inability to meet the grade requirements.
elimination of the sidewalk section for asphalt driveways.
placement of a driveway or sidewalk on City right-of-way on an unimproved street.
Other:
1 agree to the following:
1. To remove and replace this driveway to an official elevation at no additional expense to the
City of Waterloo at such time that sidewalk is constructed.
2. To remove and replace the private driveway, as needed, to an official elevation at no additional
expense to the City of Waterloo at such time that curb and gutter is constructed.
3. To pay for any additional expenses for the replacement of any such textured driveway or
sidewalk that has been removed for any City of Waterloo project.
4. To employ a bonded contractor who shall obtain a permit from the office of the City Engineer.
5. To have the driveway constructed according to the specifications and policies of the City
Engineer and under his supervision.
6. This waiver is for this property only.
Attached herewith is a payment in the amount of seven dollars ($7.00) for the purpose of
recording this agreement.

GENERAL INSTRUCTIONS TO BIDDERS
RENTAL OF FOUR (4) MOTOR GRADERS
AS PER ATTACHED SPECIFICATIONS
BID DEADLINE: 12:00 NOON, Thursday, October 19, 2017
The City of Waterloo, Iowa, is seeking bids for rental of four (4) motor graders as
described in the enclosed Specifications.
All bids are to be submitted to the City Clerk, at her office located at 715
Mulberry Street, Waterloo, Iowa 50703, in a sealed envelope, clearly
marked with the following:
a) The name and business address of the bidder
b) The specific item that is being bid
c) Date and time of the bid
All bids must meet or exceed the Specifications provided. Bids that do not meet or
exceed the Specifications will be rejected.
All bid prices are recognized by the City and the Bidder to be valid irrevocably for a
minimum of sixty (60) days from the calendar date of the Bid Opening.
Standard procedures dictate that the City of Waterloo will accept the lowest
responsive, responsible bid. However, the bidder understands that the City reserves
the right to reject any bid that is either considered not to be responsive or not most
advantageous to the City. The City will normally award the purchase contract to the
responsible Bidder who meets or exceeds the minimum specifications provided.
The City of Waterloo, Iowa reserves the right to accept or reject any and all
bids.
Upon acceptance of the bid, notification to proceed will be mailed or delivered to the
Bidder. This shall constitute acceptance of the Bid, and there, be a legal binding
agreement. The bidder guarantees the availability of said equipment for the
entire rental period as detailed in the specifications.
If applicable, the Bidder guarantees that the title conveyed for the goods shall be
delivered free from security interest or other lien or encumbrance.
The Bid price shall include all discounts, freight and/or transportation costs
necessary to supply this equipment in accordance with the specifications
and to the final destination of 625 Glenwood Street, Waterloo, Iowa.

* Rental Program is for the delivery of four (4) rental Motor Graders for a four-
month period from November 15, 2017 through March 15, 2018.
General Information: The bid should be for four (4) Motor Graders, completely serviceable with
NEW or LIKE NEW tires, meeting or exceeding the following specifications and equipment
requirements. Should be comparable to Deere 770 Series. Bids submitted should include
mounted wings.
