You are hereby ordered to make the following changes from the plans and specifications or perform
the following extra work on your contract dated October 4, 2017.
A. Description of change to be made or extra work to be done:
1. Provide and install 2 each 18" dia. and 1 each 30" dia. SW-512 area drains in the detention
area south of the large parking lot.
B. Reason for ordering change or extra work:
1. Correct drainage in this area.
C. Settlement.for cost of work to be made as follows:
1. 18"dia. SW-512 2 each at$2,250 = $4,500.00.
2. 30" dia. SW-512, 1 each at$2,750 = $2,750.00.
3. 6" dia. RF-19 sub-drain outlet, 1 each at$375.00=$375.00
4. Remove and replace rec trail paving,8 SY at$200.00=$1,600.00
5. Mobilization of equipment, Lump Sum of$2,000.00
Total Net Increase $ 11,225.00
