Q MMS Consultants, Inc. is pleased to present the following proposal for preliminary and final platting
services for the Northeast Industrial Park Plat #4 in Waterloo, Iowa. The proposed area to be

included within the platting is approximately 60 acres of City-owned property north of Martin Luther
C King Jr. Drive and south of Newell Street in Waterloo, Iowa.
A. Obtain Existing Site Information
To begin the process of preliminary platting for the Northeast Industrial Park Plat #4, the following
4 tasks will be completed by MMS Consultants staff:

• A boundary survey will be obtained of the area to be included in the proposed preliminary
a plat,
9 • Field survey work will take place to document the location of existing utilities currently in
place within the platting area,
• As-built records will be obtained for developments adjacent to the boundary of the
preliminary plat,
• As-built records will be obtained for existing roadways adjacent to the northern and
southern boundaries of the site, and
• An Iowa One-Call request will be initiated to obtain current locations and utility mapping of
N existing utilities in the area of the preliminary plat.

� B. Develop Preliminary&Final Plat Documents
Using the information collected under Item A, a preliminary and final plat will be developed for the
area. It is anticipated that the preliminary and final plats will include 4 development lots (see the
attached drawing). Two additional outlots will be created within the preliminary plat area for
0 designation as permanent public uses. These 2 outlots will consist of an area along the south side of
C Newell Street that will be used for roadway right-of-way for existing Newell Street, and the second

outlot will be used as a permanent drainage easement for the existing drainageway on the east edge
of the property.

0 Information obtained under Item A will be incorporated into the preliminary and final plat

documents for the Northeast Industrial Park Plat#4.These documents will be submitted to City staff
C for review and comment. The final submittal for work included in this agreement will be an
E approved preliminary and final plat documents for the area.

wc C. Items Not Included in This Proposal
The proposed work included in this agreement does not include development of construction
documents for improvements within established lots within the platted areas. MMS Consultants,
Inc. will provide additional services related to the platting of the Northeast Industrial Park Plat#4 as

requested by the City.

MMS Consultants, Inc. agrees to provide all labor, equipment, and materials necessary to develop
C the preliminary and final plats for the Northeast Industrial Park Plat #4 as defined above based on
time-and-materials used to complete the work. The estimated fee to complete this work is
$11,500.00.This fee is considered a maximum not-to-exceed cost.
Statements will be submitted monthly for services performed and are due upon receipt. Service
In charges of 1.5% per month will be assessed upon unpaid balance aged over 30 days.
