RESOLUTION NO. 2019-57
RESOLUTION APPROVING AN AMBULANCE SERVICES
AGREEMENT WITH UNITYPOINT AT HOME DBA
UNITYPOINT HOSPICE FOR AMBULANCE SERVICES, AND
AUTHORIZE THE MAYOR TO EXECUTE SAID DOCUMENT.
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the Ambulance Services Agreement dated January 28, 2019 with UnityPoint at Home dba
UnityPoint Hospice of Des Moines, Iowa, for ambulance services, is hereby approved and the
Mayor is authorized and directed to execute said document.
PASSED AND ADOPTED this 28th day of January 2019.
