RESOLUTION NO. 2019-129
RESOLUTION APPROVING A PARTICIPATING PROVIDER
AGREEMENT WITH IOWA TOTAL CARE, INC. OF WEST
DES MOINES IOWA, TO FACILITATE MEDICARE PATIENT
PAYMENTS, AND AUTHORIZE THE MAYOR TO EXECUTE
SAID DOCUMENT.
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the Participating Provider Agreement dated March 4, 2019, with Total Care Inc. of Des
Moines, Iowa, to facilitate Medicare patient payments, is hereby approved and the Mayor is
authorized and directed to execute said document on behalf of the City of Waterloo, Iowa.
PASSED AND ADOPTED this 4th day of March 2019.
