Change Order Proposal
Mr. Hillsman,
The following change order proposal is to provide an additional SCADA Computer in the Maintenance Garage
Office as requested in the July 11 rh, 2018 progress meeting.
Our proposal includes equipment and services as detailed below.
One (1) Desktop SCADA Computer to include Dell Precision T3620 tower, monitor, and
keyboard with specifications to match other desktop computers previously submitted.
SCADA computer will be installed on an existing desk location as designated by the owner.
Software to include Microsoft Office and Ignition Pro software as included on other
SCADA computers.
B One (1) Electrical Subcontractor Services to include installation and testing of CAT -5 ethernet
cabling between the maintenance garage telecommunications board and the new SCADA
computer location with surface mounted RJ -45 jack. The SCADA computer will utilize an
existing 120 Vac receptacle.
C One (1) Professional Services to include engineering and programming services for computer
setup, software installation, and testing.

Your net price for Items A thru C, FOB factory with freight allowed to jobsite including one (1) year
warranty from date of startup (not to exceed 18 months from date of shipment) $ 7,345.00
The above price for Items A thru C does not include any:
1. Sales or use tax.
2. New 120 Vac receptacle for the SCADA computer. An existing receptacle will be used for
the SCADA computer.
