CONTRACT WITH FIRST ADMINISTRATORS, INC. AND
o tTi s DIRECTING EXECUTION OF SAID CONTRACT BY

tr1t BE IT RESOLVED BY THE COUNCIL OF THE CITY OF WATERLOO, IOWA,
°' th4kt': the contract dated June 22, 2009, to administer the employee
n he.*1th care benefit plan for the period of July 1, 2009 through
a Jui 30, 2012, including administrative and claims services at a
cos€l' of $23 . 50 per employee per month for all three years and a
disease management addendum, by and between First Administrators,
Irk; and the City of Waterloo, Iowa, be and the same is hereby
api-ipved, and the Mayor and City Clerk authorized to execute the
same in behalf of the City of Waterloo, Iowa.
PASSED AND ADOPTED this 22nd day of June, 2009 .
