n BE IT RESOLVED BY THE COUNCIL OF THE CITY OF WATERLOO, IOWA,
°' tl t the contract dated June 22 , 2009, to provide administrative
ry and claims services for the employee dental benefit plan for the
( perjiood of July 1, 2009 through June 30 , 2012 , at a cost of $3 . 15
pex Femployee per month for FY10 , $3 . 25 per employee per month for
F?3I and $3 . 35 per employee per month for FY12, by and between
De-la Dental of Iowa and the City of Waterloo, Iowa, be and the
same, is hereby approved, and the Mayor and City Clerk authorized
to- execute the same in behalf of the City of Waterloo, Iowa.
