Professional Service Agreement

Wayne Claassen Engineering and Surveying, Inc.
PROJECT: Lot 2, Brock Third Sanitary Sewer Extension, Contract 986 Project
TO: Wayne Claassen Engineering and Surveying, Inc.
FROM: City of Waterloo, Iowa
You are hereby ordered to make the following changes to the existing Agreement for
Professional Services for the Hammond Avenue Bridge Replacement project, dated
09/03/2019
1. Add the following to said existing Agreement:
Part III. TERMS AND CONDITIONS
Article 1. ENGINEERING SERVICES
1.1 Basic Services - The Engineer agrees to perform professional services in connection
with the Project, as set forth below and contained within this Agreement:
A. Preliminary Engineering Phase
Task 5. Geotechnical Services. This task includes subcontracting two soil borings to
determine soil and groundwater conditions for the Project.
2. Settlement for cost of work to be made as follows:
Increase of $2,250.00 to the Basic Preliminary Engineering Phase of the
Agreement. The Basic Preliminary Engineering Phase of the Agreement will be
$12,250.00 with this Supplemental Agreement No. 1.
3. All other sections of said existing Agreement remain unchanged.
