Your application to the Community Foundation has been approved for funding in the amount of $7171. This grant (#20151911 — please refer to this grant number in all correspondence) was awarded through the Foundation's discretionary grant process from the fund(s) listed below. Please send any acknowledgements care of the Community Foundation. Young -Magid Fund Checks will be distributed at the award ceremony on Wednesday, January 27, 2016 at 10:00 am at the Waterloo Center for the Arts. You are encouraged to bring board members, staff/volunteers and clients to this celebratory event supporting your project. Please see the enclosed invitation for complete details. Your grant must be used in accordance with the enclosed Terns of Grant agreement. Please sign and return the agreement prior to the ceremony or hring the signed agreement to the ceremony. Please note: The Terms of Grant must be signed by the Chief Executive Officer or Board Chair of the Legal Applicant/Fiscal Sponsor. A requirement of receiving this grant is to submit a Grant Report on the full expenditure of the grant dollars by July 31, 2016. More information about this report will be included with your check. To use the funds only for the designated propose as described in the grant application and subsequent grant notification letter and not for any other purpose without prior written approval of the Community Foundation of Northeast Iowa (hereby referred to as the "Foundation"). 2. To notify the Foundation immediately of any change in (a) Grantee's legal or tax status, and (b) Grantee's ability to expend the grant for the intended purpose. 3. To maintain books and records adequate to demonstrate that the grant funds were used for the purpose for which the grant is made, and to maintain records of expenditures adequate to identify the purposes for which, and manner in which, grant funds have been expended. 4. To return to the Foundation any unexpended funds or any portion of the grant that is not used for the purposes specified herein. 5. To not provide any benefits or services to the Foundation, the donor(s) or advisor(s) to the named fund(s), or their family members as a result of this grant. 6. To allow the Foundation to review and approve the content of any proposed publicity concerning this grant prior to its release and to recognize the Foundation in all publicity materials related to the funded project or program. (Please see the appropriate "Working Together to Tell Our Stories" document on the Foundation's website.) 7. To allow the Foundation to include information about this grant in the Foundation's periodic public reports, newsletters, news releases, social media postings, and on the Foundation's website. This includes the amount and purpose of the grant, any photographs you have provided, your logo or trademark, and other information and materials about. your organization and its activities. 8. To complete the assigned Grant Report summarizing the project by the Grant Report Due Date listed above. (Note: most Grant Reports will be assigned through the Foundation's online grant site.) Interim reports may be required by the Foundation. Your reports should describe your progress in achieving the purposes of the grant and include a detailed accounting of the use and expenditure of grant funds. The undersigned hereby understands the following: 1. The Foundation reserves the right to discontinue, modify or withhold any payments under this grant award or to require a total or partial refund of any grant funds if, in the Foundation's sole discretion, such action is necessary: (a) because you have not fully complied with the terms andconditions of this grant; (b) to protect the purpose and objectives of the grant or any other charitable activities of the Foundation; or (c) to comply with the requirements of any law or regulation applicable to you, the Foundation, or this grant. 2.
