WE HEREBY PROPOSE to furnish all the materials, equipment and perform all labor for the completion of:
ASBESTOS & Lead paint INSPECTION at the above referenced location.
ASBESTOS -Inspect, collect samples, analyze, and document asbestos containing materials -including samples taken,
room location & area, type -percentage -quantity of ACM
LEAD PAINT -sampling at above locations using XRF gun as per walk through with Brian Bowman on 3/26/20. The results
will be reported in milligrams of lead per square centimeter of test area. A written report of the LBP Survey results will
be provided that will include sample locations, the XRF sampling data, and the laboratory analytical information, as
applicable. General photos of identified LBP will also be included in the report
WE PROPOSE to furnish material and labor, complete in accordance with the above specifications for the sum of:
Three Thousand Nine Hundred Eighty Dollars ($3,980.00)
TERMS: Net 10 days
All work is guaranteed to be as specified. All work to be completed in a workmanlike
manner according to specified practices. Any alteration or deviation from the above
specifications involving extra costs will be executed only upon written orders and will
become an extra charge above the estimate. All agreements contingent upon strikes,
accidents or delays beyond our control. Owner to carry fire, tornado, and other necessary
insurance. We carry general liability insurance and our workers are fully covered by
workers compensation insurance.
NOTE: This proposal may be withdrawn by us if not
accepted within 60 days.
