(NAHRO). For more than 60 years the National Association of Housing and Redevelopment Officials have fought to secure needed housing and community development programs and have represented the concerns of its members effectively. Today, NAHRO continues to champion the needs of all people to be adequately housed and for their right to live in decent neighborhoods in thriving cities and towns. NAHRO's accomplishments and services clearly demonstrate their effectiveness as a leading advocate of decent, affordable housing. The NAHRO membership dues are $1225.00 annually and may include 11 associates. The membership will be funded through the Waterloo Housing Authority's FEDERALLY FUNDED Voucher budget. Therefore, the housing authority is requesting the approval of to the board to renew the National Association of Housing Redevelopment Officials (NAHRO) Board membership. The Waterloo Housing Authority was awarded a HUD FSS grant of $35,000 to reimburse the chosen agency for Section 8 Family Self Sufficiency case Manager over the next year. The case manager assists welfare families in establishing goals and becoming economically self-sufficient under a written contract between the family and the housing authority. The request for proposals to acquire a case manager for the Family Self Sufficiency program went to the following agencies and received responses from: The Community Housing Initiatives (CHI) The Cedar Valley Community Support Services (CVCSS NA NA The Operation Threshold (O.T.) bid for the reimbursement fee of $32.10 per person each month for the first year and $33.10 per person each month the second year in the new contract. The Waterloo Housing Authority's Director is asking the Boards approval of a two-year contract with Operation Threshold to provide case management for the Family Self - Sufficiency Program. An Equal Opportunity/Affirmative Action Employer Family Self Sufficiency Service Agreement Between the Waterloo Housing Authority and Purpose: This agreement sets forth the requirements for providing case management to the Waterloo Housing Authority based on the specifications outlined in the Family Self -Sufficiency Case Management Services Proposal. Case Management services would be provided by Operation Threshold October 1, 2011 to September 30, 2013 STAFF: Beverly Cosby — FSS Case Manager Doreen Corley — Case Management Coordinator Billie Allen -Williams — Community Resources Director SERVICE TO BE PROVIDED The terms, conditions, and responsibilities of each party are as follows: 1) Operation Threshold will: effective The Agency will assign a Case Manager to work with the Family Self Sufficiency Participants 15- hours a week. The Agency Case Manager will have contact with FSS participants monthly to monitor progress, identify barriers and mediate problem solving to prevent a circumstance from jeopardizing completion of contract goals. The Agency Case Manager will determine services needed by participants and match services and providers to the participant. The Agency Case Manager will monitor service delivery of service providers and take appropriate action to ensure that service delivery is appropriate. The Agency Case Manager will counsel the FSS participants regarding compliance with public housing or Section 8 program rules and regulations as required. The Agency Case Manager will coordinate efforts and monitor participants who have elected to use the portability of their housing assistance. Housing Director will monitor the activities of the AGENCY Case Manager. The Agency Case Manager will research appropriate public and social service agencies, organizations, and private businesses to determine the services available to the family. The Agency Case Manager will be accessible to the Housing Authority as the responsible party for the provision of reports. The Agency Case Manager will notify Section 8 participants of the FSS program and request responses to determine interest, track results of recruitment, and determine eligibility of interested clients. The Agency Case Manager will maintain regular contact with the Housing Authority and make recommendations for termination of contracts where appropriate. The Case Manager maintains a full caseload, with one contact per month either by phone or in person for each of the participating individuals or families. RATE: Operation Threshold will provide these services accordingly with the following rate. YEAR 1 Unit Cost Client contact (defined as a face to face contact with the client to discuss self sufficiency goals/issues and/or process documents). $32.10 per client contact YEAR 2 Unit Cost Client contact (defined as a face to face contact with the client to discuss self sufficiency goals/issues and/or process documents). $33.10 per client contact 2) Waterloo Housing Authority will: • Provide written notice of any modification in tasks to be completed and compensate Operation Threshold based on these modifications • Identify a primary contact person to provide feedback on quality and satisfaction with work and inform the Operation Threshold contact immediately of any problems • Pay invoices for services • The Agency will enter into a contractual obligation for such services as long as both parties retain the right to terminate the contract, due to cause of convenience, after providing a 30 day notice of such intentions. COMMENTS: Operation Threshold staff has the experience necessary to provide the case management services listed in this service agreement. The Agency has been successfully providing case management services to the FSS program since October 1, 2007. Operation Threshold FSS Case Manager - Additional Services Available The FSS Case Manager being offered through Operation Threshold not only increases the services available to FSS clients, but also ensures they have information about a wide variety of other programming that can help them become and remain self-sufficient. The FSS Case Manager is part of Operation Threshold's Community Resources Department. This department includes the Family Development and Self -Sufficiency (FaDSS) program, the Tenant -Based Rental Assistance (TBRA) program and Crisis Assistance services. Services and programming available to clients served by programs in this department would also be available to FSS Clients. In addition, when the FSS Case Manager is an Operation Threshold employee the FSS Case Manager has easy access to a wide variety of expertise and information regarding other programming available at Operation Threshold. This enables the FSS Case Manager to link clients to a wide variety of programming that can help them become and remain self-sufficient, including but not limited to: • Stork's Nest - an education for incentives program working with parents from pregnancy through infants up to 13 months of age; by exhibiting healthy behaviors and by participating in educational programming to improve parenting skills, and family stability and self-sufficiency, participants earn points that are then redeemed for necessary items for their children (such as diapers, wipes and car seats). This program is on -going. • Financial Literacy Education and Counseling - Budgeting and financial literacy education will be available regularly to work with individuals and families to increase their budgeting and credit management skills, and provide anti -predatory lending education. Operation Threshold has successfully provided this service for a number of years. • Crisis and Energy Assistance - Operation Threshold provides assistance to address a variety of crises, especially with regard to utilities and rent. We administer the federal Low Income Home Energy Assistance Program (LIHEAP), which helps to defray the costs of winter heating bills. Last year, we distributed over $3 million of utility assistance —both through the regular program and to prevent utility disconnections. • WIC - is a federal program that provides nutrition education, minimal health screening, and financial support to pregnant women, infants, and children to age 5. The financial support comes in the form of checks that can only be used to purchase nutritious foodstuffs such as milk, vegetables, etc., from local vendors. Last year alone, WIC provided over $3.3 million that was used to purchase nutritious food for pregnant women, infants, and children to age 5. • Weatherization - a program that makes the homes of low income individuals and families more energy efficient (both rental and owner -occupied units). This program is funded through a combination of federal dollars and funds from utility companies. Last year we weatherized more than 280 homes. All of these Operation Threshold services are available to any eligible household, not just those in the caseload of an Operation Threshold employee. However, because the FSS Case Manager is on staff and attends many of our internal and external trainings, s/he is more aware of the inter -workings of the services that are available; thus, the FSS Case Manager can make more effective linkages with our other programs and services. The Waterloo Housing Authority (WHA) received notice of a Capital Fund grant of $50,825.00 through a letter on July 2011. The purpose of the funding is to perform modernization projects on Ridgeway Towers, examples of these improvements are: roof, bathroom, kitchen cabinets, etc. However, the method of funding is a formula allocation. Waterloo Housing Authority's share was calculated on the basis of current information available in HUD's data system regarding the age of the units, the number of bedrooms and the total number of units. The application and budget submitted can be revised at any time during the next couple of years and need to be expended during the next four years for a range of projects from operating expenses at Ridgeway Towers (administration, operations, etc and other projects). The Housing Authority is required to conduct meetings between the Housing residents and staff, conduct an audit within nine months of their fiscal year and need to develop a HUD required Housing Plan between November and April of each year. With the help of the tenants the current Capital Fund Program and future funding priorities is a part of the planning process. The Waterloo Housing Authority's Director is seeking board approval of the 2011 Capital Fund Grant. Note attachment of housing PHAS Score. An Equal Opportunity/Affirmative Action Employer Capital Fund Program U.S. Department of Housing (CFP) Amendment and Urban Development To The Consolidated Annual Contributions Office of Public and Indian Housing Contract (form HUD-53012) Whereas, (Public Housing Authority) Waterloo Housing Authority (IA050 ) (herein called the "PHA") and the United States of America, Secretary of Housing and Urban Development (herein called "HUD") entered into Consolidated Annual Contributions Contract(s) ACC(s) Numbers(s) KC-3047 dated 11/15/1995 Whereas, HUD has agreed to provide CFP assistance, upon execution of this Amendment, to the PHA in the amount to be specified below for the purpose of assisting the PHA in carrying out capital and management activities at existing public housing developments in order to ensure that such developments continue to be available to serve low-income families. HUD reserves the right to provide additional CFP assistance in this FY to the PHA. For Non -qualified PHAs: (i) In accordance with the HUD regulations, the Annual HA Plan has been adopted by the PHA and approved by HUD, and may be amended from time to time. The capital and management activities shall be carried out as described in the CFP Annual Statement/Performance and Evaluation Report (HUD-50075.1). For Qualified PHAs: (i) The CFP Annual Statement/Performance and Evaluation Report (HUD-50075.1) has been adopted by the PHA and verified by HUD. The capital and management activities shall be carried out as described therein. OR (ii) If the CFP Annual Statement/Performance and Evaluation Report has not been adopted by the PHA and/or verified by HUD, the PHA may use its CFP assistance under this contract for work items contained in its approved CFP 5-Year Action Plan (HUD-50075.2), before the CFP Annual Statement/Performance and Evaluation Report is adopted by the PHA and verified by HUD. For cases where HUD has approved a Capital Fund Financing Amendment to the ACC (CFF Amendment attached), HUD will deduct the payment for amortization scheduled payments from the grant immediately on the effective date of this CFP Amendment. The payment of CFP funds due per the amortization scheduled will be made directly to a designated trustee (Trustee Agreement attached) within 3 days of the due date. Regardless of the selection above, the 24 month time period in which the PHA must obligate this CFP assistance pursuant to section 9(j)(1) of the United States Housing Act of 1937, as amended, (the "Act") and 48 month time period in which the PHA must expend this CFP assistance pursuant to section 9(j)(5) of the Act starts with the effective date of this CFP amendment (the date on which CFP assistance becomes available to the PHA for obligation). Any additional CFP assistance this FY will start with the same effective date. 4. Subject to the provisions of the ACC(s) and paragraph 3. and to assist in the capital and management activities, HUD agrees to disburse to the PHA or the designated trustee from time to time as needed up to the amount of the funding assistance specified herein. 5. The PHA shall continue to operate each development as low-income housing in compliance with the ACC(s), as amended, the Act and all HUD regulations for a period of twenty years after the last disbursement of CFP assistance for modernization activities for any public housing or portion thereof and for a period of forty years after the last distribution of CFP assistance for development activities for any public housing and for a period of ten years following the last payment of assistance from the Operating Fund to the PHA. However, the provisions of Section 7 of the ACC shall remain in effect for so long as HUD determines there is any outstanding indebtedness of the PHA to HUD which arose in connection with any development(s) under the ACC(s) and which is not eligible for forgiveness, and provided further that, no disposition of any development covered by this amendment shall occur unless approved by HUD. 6. The PHA will accept all CFP assistance provided for this FY. If the PHA does not comply with any of its obligations under this Amendment and does not have its Annual PHA Plan approved within the period specified by HUD, HUD shall impose such penalties or take such remedial action as provided by law. HUD may direct the PHA to terminate all work described in the Capital Fund Annual Statement of the Annual PHA Plan. In such case, the PHA shall only incur additional costs with HUD approval. 7. The Waterloo Housing Authority Director received the following estimate to install a telephone line for the elevator at the Ridgeway Towers building: Previously a couple of the tenants have been considerate enough to let us utilize their telephone lines for the elevator phone. Housing has requested a bid from Qwest to install and activate the line into the D-Marc and to take the line from the D-Marc to elevator. The request to the vendor was to provide the service and the equipment that's needed to install the line. Scope of the work: Qwest will install a new line to the De -Marc if necessary; hook up line to the elevator. Labor, estimate 1 hour@ ($90/hour Service order charge(a $50 for a total of approximately $140. Service charge will be $35.00 per month for the phone line. The Waterloo Housing Authority is requesting the authorization of the board to approve the following provider and prices to install and hook and activate the line to the elevator at Ridgeway Towers. The funds for the line will come from The Ridgeway Towers operation Budget for the building telephone faxes and expense line item. A couple of the apartment showers are leaking out into the common area and into the first floor hallway. After researching it has been found that there is nothing at the bottom of the wall to stop the water from going beyond the shower area once the grout starts to deteriorate when the bottom grout area of the floor to wall cracks. There was never a waterproof liner on the floor to wall; by retiling and re -grouting the wall and floor this will keep the water from going under the wall. Maintenance has patched the leak temporarily. The Waterloo Housing Director is requesting the board's approval to seek bids to install liners in all of the handicapped showers.
