HomeMy WebLinkAboutAECOM-3/14/2016 Midport Blvd ExtensionACOM AECOM 319-232-6531 tel 501 Sycamore Street 319-232-0271 fax Sulte 222 Waterloo, Iowa 50703 www.aecom.Gom MIDPORT BOULEVARD EXTENSION CONSTRUCTION -REVIEW SERVICES CITY OF WATERLOO, IOWA SUPPLEMENTAL AGREEMENT NO. 1 WHEREAS, a Professional Services Agreement was entered into between the City of Waterloo, Iowa (Client), and AECOM Technical Services, Inc. (ATS), ofWaterloo, Iowa, dated August 3,2015, for the design survey, preliminary design, and final design for the Midport Boulevard Extension from the existing end of pavement near ConAgra Foods to the existing end of pavement north of Airline Highway; and WHEREAS, the Client and ATS now desire to enter mb Supplemental Agreement No. 1 for Phase 11, Construction -Review Services, for that portion of the project includirtg the FY 2016 Midport Boulevard Extension, Phase 111. NOW THEREFOF1E, it is mutually agreed to amend the original Professional Services Agreement as follows: Project Description The project consists of construction of the Midport Boulevard Extension from the existing end of pavement near the east end of the ConAgra Foods current facility to the existing end of pavement north of Airline Highway, a distance of approximately 1,700 linear feet. The project will complete the connection between Airport Boulevard and Airline Highway in the Midport America Park. The project includes approximately 1,700 L.F. of roadway paving. The roadway will be designed and graded for a boulevard section that could be extended north and west to existing Midport Boulevard. Paving, however, will only be completed for the south two lanes of the roadway. 11. Scope of Services The Scope of Services will encompass and include services, materials, equipment, personnel and supplies necessary to provide construction staking, field review, materials testing and contract administration during the construction phase for the project defined above. The Scope of Services is further defined as foliows: Task 1. Conduct a preconstruction conference attended by representatives of the Contractor, Client, Consultant and affected utilities. Task 2. Provide construction -staking for horizontal and vertical controls for the project as follows: a. Set Project Control b. Set Stakes for Roadway Grading c. Check Stakes on Subbase d. Set Stakes for Storm Sewer e. Set Stakes for Subdrain f. Set Stakes for Culverts g. Set Stakes for Drives h. Set Stakes for Paving at 25 -Foot Intervals CCOM 1• Stake Sign Locations Set Stakes for Silt Fence Page 2 Task 3. Review shop drawings and other submittals as required oI the Contractor by the contract documents for conformance with the design concept of the project and compliance with the information given in the contract documents. Task 4. Answer design interpretation questions from the Client, Contractor, review staff and appropriate agencies. Task 5. Prepare bi-weekly applications for payment based on information provided by field review staff and Contractor and forward to the Client for execution with recommendation for approval and payment. Task 6. Perform construction site visits by design personnel at appropriate stages of construetion to review the quality 01 the work and to determine whether the work conforms to the contract documents. Task 7. Prepare and assist the Client and Contractor in processing contract change orders. Task 8. Provide periodic field observation during construction to review the work of the Contractor to determine if the work is proceeding in general accordance with the contract documents and that completed work appears to conform to the contract documents. Staffing requirements may be adjusted during the project in relation to the level of construction activity. Task 9. Provide weekly SWPPP reviews along with City of Waterloo and contractor personnel, as required by lowa DNR and City 01 Waterloo. Task 10. Report to the Client any work believed to be unsatisfactory, faulty or defective or does not conform to the contract documents, and advise the Client of any work that should be corrected or rejected. Task 11. Consider and evaluate Contractor's suggestions for modifications and repori them with recommendations to the Client. Task 12. Participate in a review of the project with the Client and review staff near project completion and prepare a list of items to be completed or corrected. Task 13. Participate in a field observation of the completed project with the Client and review staff before a final application for payment is processed for the Contractor. Task 14. Maintain 1 iles for correspondence, reports of the job conferences, shop drawings and sample submissions, reproductions of original contract documents including addenda, change orders, field modifications, additional drawings issued subsequent to the execution of the contract, Engineer clarifications and interpretations 01 the contract documents, progress reports and other project -related documents. Task 15. Provide the Client with a copy of revised drawings of the construction plans (record drawings) for the project based on the construction obseniation records of the field review staff and the Contractor showing those changes made during construction considered significant. Page 3 A= OM Task 16. Prepare and assist the Client with the final close-out documentation received from the Contractor. III. Compensation Compensation for the above services will be on an hourly basis in accordance with Part VI of the original agreement. Estimated fee for the services is Eighty Thousand Dollars ($80,000.00), and shall not be exceeded without authorization of the Client. IV. In all other respects, the obligations of the Client and the Consultant shall remain as specified in the Professional Services Agreement dated August 3, 2015.
