2016 GATEWELL REPAIRS
CONSTRUCTION -RELATED SERVICES
SLJPPLEMENTAL AGREEMENT NO. 1
WHEREAS, a Protessional Services Agreement was entered into between the City ot Waterloo,
lowa (Client), and AECOM Technical Services, Inc. (ATS) af Waterloo, lowa, dated August 10,
2015, tor the design survey and final design tor the 2016 Gatewell Repairs Project in Waterloo,
lowa; and
WHEREAS, the Client and ATS now desire to enter into Supplemental Agreement No. 1 for
construction -related services for the 2016 Gatewell Repairs Project.
NOW THEREFORE, it is mutually agreed to amend the original Protessional Services Agreement
as toliows:
Project Description
The project consists of the removal and replacement of tour (4) gate vaives, which outlet
into Black Hawk Creek. The project includes removal and replacement ot existing gatewell
equipment and operators, storm sewer connections, trattic control and other incidental
items tor the 2016 Gatewell Repairs Project.
11. Scope ot Services
The Scope of Services will encompass and include services, materia!s, equipment,
personnel and supplies necessary to provide field review, materials testing and contract
administration during the construction phase tor the project defined above. The Scope ot
Services is turther detined as foliows:
Task 1. Conduct a preconstruction conference attended by representatives ot the
Contractor, Client, Consultant and attected utilities.
Task 2. Review shop drawings and other submittals as required ot the Contractor by the
contract documents tor conformance with the design concept ot the project and compliance
with the information given in the contract documents.
Task 3. Answer design interpretation questions from the Client, Contractor, review staff
and appropriate agencies.
Task 4. Prepare bi-weekly applications tor payment based on intormation provided by field
review staff and Contractor and torward to the Client for execution with recommendation tor
approval and payment.
Task 5. Perform construction site visits by design personnel at appropriate stages ot
construction to review the quality of the work and to determine whether the work contorn,s
to the contract documents.
Task 6. Prepare and assist the Client and Contractor in processing contract change orders.

Task 7. Provide periodic field observation during construction to review the work of the
Contractor to determine if the work is proceeding in general accordance with the contract
documents and that completed work appears to contorm to the contract documents.
Staffing requirements may be adjusted during the project in relation to the level of
construction activity.
Task 8. Report to the Client any work believed to be unsatisfactory, faulty or defective or
does not contorm to the contract documents, and advise the Client of any work that should
be corrected or rejected.
Task 9. Consider and evaluate Contractor's suggestions for modifications and report them
with recommendations to the Client.
Task 10. Participate in a review of the project with the Client and review staif near project
completion and prepare a Iist ot items to be completed or corrected.
Task 11. Participate in a tield observation of the completed project with the Client and
review staff betore a final application for payment is processed for the Contractor.
Task 12. Maintain files tor correspondence, reports of the job conferences, shop drawings
and sample submissions, reproductions of original contract documents including addenda,
change orders, field modifications, additional drawings issued subsequent to the execution
of the contract, Engineer clarifications and interpretations of the contract documents,
progress reports and other project -related documents.
Task 13. Provide the Client with a copy ot revised drawings of the construction plans
(record drawings) for the project based on the construction observation records of the field
review staft and the Contractor showing those changes made during construction
considered significant.
Task 14. Prepare and assist the Client with the tinal close-out documentation received froni
the Contractor.
111. Compensation
Compensation for the above services will be on an hourly basis in accordance with Part Vl
of the original agreement. Total compensation is an estiniated fee of Fifty -Seven Thousand
Nine Hundred Dollars ($57,900.00).
IV. ln all other respects, the obligations of the Client and ATS shall remain as specified in the
Protessional Services Agreement dated August 10, 2015.
