structures located at: 312 Charles Street, 318 E. 9th Street, 332 Irving Street, 720 West Park
Avenue and 116 West Parker Street; and authorize Mayor to execute said document —Submitted
by Louis Starks, Contracts and Grants Coordinator.

approval of plans and specifications for removal of asbestos from six (6) structures located at:
915-1917 Lafayette Street, 919 Lafayette Street, 1013 Lafayette Street, 1021 Lafayette Street,
706 W. 1st Street; and 927 Lafayette Street as an alternate —Submitted by Louis Starks,
Contracts and Grants Coordinator.
Moved by Welper, seconded by Greenwood to Approve request by Jay Koweil to purchase seven
(7) signs remaining at the former Grand Hotel site for a total of $40.00—Submitted by Noel
Anderson, Community Planning & Development Director.
With no fiirther business before the Council, it was moved by Welper, seconded by Greenwood

Summary Statement: Staff received three bids as follows:
Evans Inspection Services, Inc.: $2,550.00,
VACT Associates, Inc. $3,450.00, and
Trilead Corp: $6,092.80
Staff is recommending Award to Evans Inspection Services, Inc.
Expenditure Required: $2,550.00
Source of Funds: G.O. funding for all structures except 332 Irving St. EDI funding for
332 Irving St., located in the Brownfield Area.
Policy Issue: Slum and Blight, Public Safety, Housing, New Construction
Alternative: None. Abatement before demolition required by State and Federal law.

AGREEMENT FOR ASBESTOS SURVEY AND REPORT
FOR CDBG GROUP NO. 140-AS-02-28-11-GO
THIS AGREEMENT made this 28th day of February 2011, by and between
EVAN'S INSPECTION SERVICES, INC., a corporation organized and existing under
the laws of the State of Iowa, hereinafter called the "CONTRACTOR", and the CITY OF
WATERLOO, IOWA, acting through its COMMUNITY DEVELOPMENT BOARD,
hereinafter called the "CITY".
WITNESSETH, that the Contractor and the City for the consideration stated herein
mutually agree as follows:
ARTICLE 1. STATEMENT OF WORK. The Contractor shall furnish all supervision,
technical personnel, labor, materials, machinery, services, and perform and complete
within 30 calendar days after receipt of Notice to Proceed, all work required for the
inspection and detection of Asbestos in an efficient and workmanlike manner, as required
by the Iowa Department of Labor and the U.S. Environmental Protection Agency
regulations.
ARTICLE 2. THE CONTRACT PRICE. The City will pay the Contractor for the
performance of the Contract in current funds, subject to additions and deductions as
provided in the Section "Changes in the Work" under General Conditions, Part I, and
subject to all other conditions set forth in the Contract Documents, the sum of
TWO THOUSAND, FIVE HUNDRED FIFTY AND NO/100 DOLLARS ($2,550.00),
and in accordance with the contractor's bid.
ARTICLE 3. CONTRACT. The executed Contract Documents shall consist of the
following:
a. This Agreement
b. Addenda
c. Invitation to Bid
d. Instructions to Bidders
e. Signed copy of Bids.
THIS AGREEMENT, together with the other documents enumerated in this Article 3,
which said other documents are as fully a part of the Contract as if hereto attached or
herein repeated, forms the Contract between the parties hereto. In the event that any
provision in any component part of this Contract conflicts with any provisions of any
other component part, the provision of the component part first enumerated in this Article
3 shall govern, except as otherwise specifically stated.
ARTICLE 4.
IN WITNESS WHEREOF, the parties hereto have caused this Agreement to be executed
in three (3) original copies on the day and year first above written.

preparation for the demolition of the structure and site clearance. It is anticipated that new
single-family housing will be constructed at the Lafayette St. sites using federal funds.
Expenditure Required: Estimate: $13,500.00. It is not anticipated that a public hearing
will be required.
Source of Funds: Federal EDI for all structures except 706 W. 1s` St., General Obligation
Funds for that structure.
Policy Issue: Slum and Blight, Public Safety, New Housing
Alternative: None. Removal required by State and Federal law.
Background Information: City acquired most of these lots by either purchase or using

3. Sufficient time should be allowed for mailed bids to be delivered by normal postal
operation. Late bids will not be considered. Bids must be received by 3:30 p.m.,
March 14, 2011 in the City Clerk's office. Bids will be opened at the City
Council regular meeting of Finance Building and Grounds Committees at 5:30
p.m.
4. Asbestos abatement shall be completed within Ten (10) working days of Notice to
Proceed (NTP).
TIME IS OF THE ESSENCE and default requirements will be strictly adhered to. Failure to
complete the project as set forth herein, shall constitute a material default and the City may
immediately hire another contractor to complete work and deduct any costs associated from
the original contract price associated with the contract substitution. Execution of this
Contract requires flexibility and the ability to perform under extraordinary conditions.
5. Your bid proposal shall include spaces for authorized signature(s) of the City's
representative, to be signed if the lowest responsible bid. Submit proposals in
duplicate. Bids to be awarded at the City Council Meeting March 21, 2011.
6. Enclosed is a list of asbestos items for each property. Remove only areas
indicated on the report as having asbestos in the structure.
7. Please pay attention to the Asbestos Inspection Report's Notes &
Recommendations.
8. Proposals should include (1) OSHA required personal and excursion air
monitoring, and (2) work performed in accordance with EPA-EHERA,
NESHAP, Iowa DNR and Iowa DNR regulations.
