RESOLUTION NO. 2020-354
RESOLUTION APPROVING RENEWAL OF CONTRACT WITH
NATIONAL INSURANCE SERVICES FOR ADMINISTRATION OF
THE EMPLOYEE LIFE AND LONG TERM DISABILITY BENEFIT
PLAN FROM JULY 1, 2020 THROUGH JUNE 30, 2021, AND
AUTHORIZING THE HUMAN RESOURCES DIRECTOR TO
EXECUTE SAID DOCUMENT.
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the contract dated May 18, 2020 for the renewal of contract between National Insurance
Services and the City of Waterloo, Iowa, for administration of the Employee Life and Long Term
Disability Benefit Plan from July 1, 2020 through June 30, 2021, is hereby approved and the
Human Resources Director is authorized and directed to execute the same on behalf of the City
of Waterloo, Iowa.
PASSED AND ADOPTED this 18th day of May 2020.
