RESOLUTION NO. 2012-137
RESOLUTION APPROVING AMBULANCE SERVICES
AGREEMENT WITH ALLEN HOSPICE OF WATERLOO,
IOWA AND DIRECTING EXECUTION OF SAID
AGREEMENT BY MAYOR.
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the Ambulance Services Agreement dated February 13, 2012, to
provide medical transportation of Hospice patients to the Hospice
facility, for the period April 1, 2012 to April 1, 2013, by and
between Allen Hospice of Waterloo, Iowa and the City of Waterloo,
Iowa, be and the same is hereby approved, and the Mayor
authorized to execute the same in behalf of the City of Waterloo,
Iowa.
PASSED AND ADOPTED this 13th day of February, 2012.
