You are hereby ordered to make the following changes from the plans and specifications or perform the
following extra work on your contract dated Tune 8,2020.
A. Description of change to be made or extra work to be done:
1. Add storm sewer and fill in ditch on south side of Newell Street on east end
B. Reason for ordering change or extra work:
Reduce future maintenance issues in the ditch with continuous water coming from storm basin on Con-
trol site.
C. Settlement for cost of work to be made as follows:
1. Storm Sewer,RCP,24" 326 LF $79.20/LF $25,819.20
2. Remove and Reinstall 24" Apron 1 EACH $2,420/EACH $2,420.00
3. SW-401 Manhole w/ SW-601C Cover 1 EACH $3,850/EACH $3,850.00
4. Storm Sewer,RCP,18" -52 LF $58.00/LF -$3,016.00
5. SW-401 Storm Manhole -1 EACH $2,900/EACH -$2,900.00
Total Net Increase $26,173.20
