PROFESSIONAL SERVICES AGREEMENT AMENDMENT NO. 6
THIS AMENDMENT, made this 15th day of March 2021 by and between, City of Waterloo the CLIENT,
and HR GREEN, INC. (hereafter "COMPANY'), for professional services concerning:
Shaulis Road Additional Design Effort.
hereby amends the original Professional Services Agreement dated April 6, 2020 as follows:
The CLIENT and COMPANY agree to amend the Scope of Services of the original Professional
Services Agreement and previous amendments as follows:

The CLIENT and COMPANY agree to amend other provisions of the original Professional Services
Agreement and previous amendments as follows:
N/A
In consideration for these services, the CLIENT AGREES to adjust the payment for services performed
by COMPANY on the following basis:
® Per current rate schedule with the maximum fee to be increased by eleven thousand, five
hundred Dollars ($ 11,500.00)
The total authorized compensation after this Amendment, including the original Professional Services
Agreement and all previous Amendments, is seven hundred thirty-eight thousand, two hundred twenty
Dollars and seventy-five Cents ($ 738,220.75)
THIS AMENDMENT is subject to all provisions of the original Professional Services Agreement.
THIS AMENDMENT, together with the original Professional Services Agreement and all previous
amendments represents the entire and integrated AGREEMENT between the CLIENT and COMPANY.
THIS AMENDMENT executed the day and year written above.

ATTACHMENT A - SCOPE OF SERVICES
The following tasks will be added to the Scope of Services to be performed by the COMPANY:
Due to the desire to include drainage easements/exhibits and coordination with Iowa DNR,the following tasks will
accomplish this goal.
Task P— DRAINAGE EASEMENT
The COMPANY will provide Permanent Easements delineating the flood plain area and coordination with the Iowa
DNR. Refer to Vobr Neimeyer, LLC Professional Surveying for ROW platting services.

Vobr Niemeyer, LLC is pleased to submit you a quote for the topographic survey and Right-
of-way for the above referenced project. Survey services shall include OSHA 10 hour
qualified personnel and licensed land surveyor in responsible charge personnel, equipment,
and supplies to perform the following:
Scope of Services
Permanent Easement Platting
• Prepare up to 4 Permanent Easements, to included acquisition plat and legal
description.
The total cost not to exceed for Permanent Easement Platting...............................$4,150
Easement Corner Monumentation
• Set permanent easement corner monuments for up to 4 Permanent Easements
acquisitions.
The total cost not to exceed for Easement Corner Monumentation________________________$1,850
Schedule
We propose to begin work on this project immediately following acceptance of this proposal.
Only the items specifically listed in this quote will be delivered for the fee shown. Any other
items not listed above that you would request services for could be performed at our standard
hourly rate, or identified and negotiated into a revised lump sum fee.
This estimate is good for 30 days. If acceptable, please contact us for a standard purchase
order for execution. We look forward to working with you on this project.
