RESOLUTION NO. 2021-331
RESOLUTION APPROVING RENEWAL OF CONTRACT WITH
WELLMARK BLUE CROSS BLUE SHIELD OF IOWA FOR
ADMINISTRATION OF THE EMPLOYEE HEALTH CARE AND
PRESCRIPTION PLAN FROM JULY 1, 2021 THROUGH JUNE 30,
2022, INCLUDING ADMINISTRATIVE FEES AND STOP LOSS
INSURANCE IN THE AMOUNT OF $1,769,013, AND AUTHORIZING
THE HUMAN RESOURCES DIRECTOR TO EXECUTE SAID
DOCUMENT.
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the contract dated May 18, 2020, for the renewal of contract between Wellmark Blue Cross
Blue Shield of Iowa and the City of Waterloo, Iowa, for administration of the Employee Health
Care and Prescription Plan from July 1, 2020 through June 30, 2021, including administrative
fees and stop loss insurance in the amount of $1,769,013, is hereby approved and the Human
Resources Director is authorized and directed to execute the same on behalf of the City of
Waterloo, Iowa.
PASSED AND ADOPTED this 17th day of May 2021.
