RESOLUTION NO. 2000-362
RESOLUTION APPROVING AGREEMENT WITH PATHWAYS
BEHAVIORAL SERVICES, INC. AND DIRECTING
EXECUTION OF SAID AGREEMENT BY MAYOR.
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the Agreement dated June 26, 2000 to provide City' s Employee
Assistance Program (EAP) for F.Y. 2001 at a cost of $14 .50 per
employee for an estimated total cost of $8, 400. 00, by and between
Pathways Behavioral Services, Inc. and the City of Waterloo, Iowa,
be and the same is hereby approved, and the Mayor authorized to
execute the same in behalf of the City of Waterloo, Iowa.
PASSED AND ADOPTED this 26t'' day of June, 2000.
