RESOLUTION NO. 1999-438
RESOLUTION APPROVING RFPS FOR HEALTH, DENTAL,
LIFE AND DISABILITY INSURANCE FOR THE CITY OF
WATERLOO, IOWA.
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the RFPs for Life Insurance with Schools Insurance, Dental
Insurance with Delta Dental, Health Insurance Self Funded with
Wellmark Administrator and Health Insurance-HMO with John Deere
Clinic and John Deere Heritage, be and the same are hereby
approved and City Clerk authorized to enter into contracts with
said companies.
PASSED AND ADOPTED this 28th day of June 1999 .
