Description of Change
4a Provide polyvinyl chloride (PVC) domestic water pipe ADD $0.00
instead of copper, and replace copper pipe previously
installed in Structure 180 with PVC pipe in accordance
with the enclosed Cost Proposal Request (CPR) 010
dated April 16, 2021.
4b Provide lintel for louver 330-03 in Structure 330 in ADD $1,895.00
accordance with the enclosed CPR 012 dated
May 20, 2021, and cost proposal dated June 1, 2021.
4c Amend Contract Substantial Completion date and ADD $0.00
Contract Final Completion date for time lost to wet
weather between May 1, 2020 and July 1, 2020, as
summarized in the enclosed letter from
Woodruff Construction dated July 7, 2021.
4d Provide monorail crane modifications in Structure 180 ADD $2,435.00
in accordance with the enclosed cost proposal dated
July 14, 2021.
4e Provide rotating disc gate valve on the existing 4-inch ADD $2,928.00
W2 line in Structure 180 in accordance with the
enclosed cost proposal dated July 16, 2021.
4f Provide additional door removal and replacement in ADD $16,759.00
Structures 240 and 330 in accordance with the enclosed
CPR 01 1 dated July 23, 2021 and cost proposal dated
July 28, 2021.
