SUPPLEMENTAL AGREEMENT NO. 1
RELEASE OF AIRPORT PROPERTY
WATERLOO REGIONAL AIRPORT
WATERLOO,IOWA
WHEREAS,a Professional Services Agreement was entered into between the City of Waterloo,Iowa, (Client)and
Earth Tech,Inc.,of Waterloo,Iowa,dated July 6,2004,to provide assistance and prepare documents for the release
of airport property as defined by the Airport Board and the City of Waterloo; and
WHERAS,the Client and Consultant desire to amend the previous agreement to include cost of appraisal,provide a
Phase I Cultural Resource Survey and allow for additional requests from the Federal Aviation Administration.
NOW THEREFORE, it is mutually agreed to amend the original Professional Services Agreement as follows:
I. SCOPE OF SERVICES
The Scope of Services is modified to include the following:
A. Appraisal. Provide a narrative appraisal report-a Summary Report Format of a Complete Appraisal
Assignment as per the current Uniform Standards of Professional Appraisal Practice for two parcels
in the project area. These services will be provided by Valuation Services,Inc.,as a subconsultant.
B. Cultural Resources Survey. Provide a Phase I Cultural Resources Survey for approximately 311
acres in the project area. This task includes:
• Review of archaeological and historical records
• Field investigation including shovel testing or bucket augering in locations with deep site
potential
• Geoarchaeological assessments
• Preliminary evaluation of archaeological sites if they are encountered
• Preparation of a report that will meet the guidelines established by the Secretary of Interior and
the State Historical Society of Iowa.
Phase H testing of any identified sites is not included in this task. This task will be provided by Bear
Creek Archeology,Inc., as a subconsultant.
C. FAA Coordination. This task includes coordination with FAA on the above task as well as other
items of information that have been or may be requested by FAA for the property release. Since the
level of effort for this task can not be anticipated,it is restricted by the staff hours estimated at time
of agreement.
II. COMPENSATION AND PAYMENT
Compensation estimated in the original agreement shall be increased by Ten Thousand Dollars($10,000.00).
III. In all other respects,the obligations of the Client and Earth Tech shall remain as specified in the Professional
Services Agreement dated June 6, 2004.
IN WITNESS WHEREOF, the parties hereto have executed this Supplemental Agreement No. I as of the dates
shown below.
