SPECIAL CONDITIONS • A portion of this award is pending approval by the U.S. Department of Justice. Should 2006 COPS Methamphetamine Hot Spots funding be reduced or limited by the Department of Justice, this grant award may be adjusted. SIGNATURES/DATES Legal Appl' ant/Date 0 Program Director/Date ODCP Adm strator/Date ASO Alts`:, `>`oncepts,, LLC ADDENDUM ONE It is hereby understood and agreed that this Addendum attach to and become a part of the Claims Service Contract which took effect on July 1, 2006 between ALTERNATIVE SERVICE CONCEPTS, LLC ("ASC") and CITY OF WATERLOO, IOWA ("Client"), as though fully repeated and set herein. "Client" agrees to pay "ASC" based on the attached Fee Schedule for the administration of the Workers' Compensation claims that are reported by "Client" to "ASC" with dates of loss from January 1, 1952 through June 30, 2006.
