this grdoes not constitute approval of waiver from any Federal or state statutory/regulatory requirements for a United States
Departgrant. The grantee agrees to perform all services andfurnish all supplies setforth in the application of this grant awardfor the
consideration stateerein. This grant consists of the application for funds, the grant award notice, the budget documents, the standard grant
conditions, the reporting forms,and all approved grant revision documents. All parties to this grant award acknowledge that they have fully read and
understand this contract,and agree to abide by the terms set forth within.
SPECIAL CONDITIONS
• A portion of this award is pending approval by the U.S. Department of Justice. Should 2006 COPS Methamphetamine Hot Spots funding be reduced
or limited by the Department of Justice, this grant award may be adjusted. You will be notified when a final award is received from the U.S.
Department of Justice

ADDENDUM ONE
It is hereby understood and agreed that this Addendum attach to and become a
part of the Claims Service Contract which took effect on July 1, 2006 between
ALTERNATIVE SERVICE CONCEPTS, LLC ("ASC") and CITY OF WATERLOO,
IOWA ("Client"), as though fully repeated and set herein.
"Client" agrees to pay "ASC" based on the attached Fee Schedule for the
administration of the Workers' Compensation claims that are reported by "Client" to
"ASC" with dates of loss from January 1, 1952 through June 30, 2006.
FEE SCHEDULE
Proposal for 411 Duty Related Retirees
Runoff claims will be handled on a per-claimant, per-year fee basis at the following rate:

Should the City wish for ASC to manage the claims payment account, an additional fee
will apply.
Allocated Expenses
Allocated expenses will be charged to the claim file and include fees for:
• Legal services • Professional photographs
• State-mandated EDI • Official documents and transcripts
• Court reporters • Experts' / rehabilitation services

Claims Handling at Contract Conclusion
At the conclusion of the contract, the following options are available for continued
handling of open claims:
• Annual per-claimant fee at ASC's prevailing rates
• Claims returned to the City
All other terms, conditions, and agreements contained in said Contract remain in
full force and effect.
