RESOLUTION NO. 2012-676
RESOLUTION APPROVING ONE-YEAR RENEWAL
AGREEMENT OF STOP LOSS HEALTH INSURANCE WITH
WELLMARK BLUE CROSS BLUE SHIELD.
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the one-year renewal agreement, dated June 25, 2012, to
provide stop loss health insurance for a premium of $754,068.00,
for the period of July 1, 2012 through June 30, 2013, be and the
same is hereby approved, and the City Clerk is authorized to
execute said document on behalf of the City of Waterloo, Iowa.
PASSED AND ADOPTED this 25th day of June, 2012.
