RESOLUTION APPROVING F.Y. 2013 CLAIMS SERVICE
CONTRACT AGREEMENT FOR THIRD PARTY
ADMINISTRATION.
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the R.Y. 2013 Claims Service Contract Agreement for Third
Party Administration between Alternative Service Concepts and the
City of Waterloo, in an amount not to exceed $23,670.001, plus
bill review, to provide administrative and claims services for
self -funded work comp insurance, is hereby approved, and the City
Clerk is authorized to execute said document on behalf of the
City of Waterloo, Iowa.
PASSED AND ADOPTED this 25th day of June, 2012.
