Our bid price shall remain in effect for a period of forty-five (45) calendar days and is based on fuel at $2.59/gallon. There may be a fuel surcharge if prices escalate in an unforeseen manner: SCOPE OF WORK: Removal and disposal of the following asbestos containing materials as per the specifications of Ames Environmental: 1) Approximately 80 sq feet of boiler insulation in basement 2) Approximately 350 linear feet of pipe insulation in basement 3) Approximately 120 linear feet of black cork wrap in basement 4) Approximately 25 sq feet of stone patterned linoleum from E. first floor apartment bathroom 5) Approximately 100 sq feet of linoleum and mastic from the upstairs W. apartment kitchen 6) Approximately 12 sq feet of brown glue spot from upstairs SW bathroom ceiling 7) Approximately 435 sq feet of roofing and roof flashing from the west porch and south front porch 8) Approximately 18 sq feet of 9"x9" floor the and mastic from up NE room closet 9) Approximately 225 sq feet of 12"x12" floor tile and mastic from the W apartment first floor kitchen Our bid includes the costs of providing, over and above the Scope of Work: • Notification to proper regulatory agency for asbestos removal only, • Disposal of asbestos at an approved landfill, • Insurance per State requirements, and • Project documentation, submitted at time of invoice. • 6 years record keeping for the State of Iowa. Our bid does not include regulatory notification for asbestos removal or other work prior or subsequent to our work. This is not a proposal to remove all asbestos from the area. Hidden, obscure and/or encapsulated asbestos containing material or debris (includes residue) of any kind or nature is not included, however, the removal as described in the "Scope of Work" shall be performed to your satisfaction. The owner should expect that since tape and polyethylene sheeting are used that some paint may be removed with tape removal or tape residue left and is to be expected. Further, since copious amounts of water are used some leaks may occur and this is also to be expected. The contractor will make every effort to minimize these events. By agreement of the parties through acceptance of this bid, Affordables' regulated responsibility will terminate once we have left the work site, after which time the Property Owner shall be responsible for prior and subsequent notices, administrative orders and penalties,reports, permits and/or inspections by law or regulation related to renovation and/or demolition of the property. Further,this bid assumes that the owner will provide water and power so the work may be performed. An additional charge may accrue if these services have to be purchased by the contractor to perform the work. Payment is due in full immediately upon completion of the asbestos removal portion of the project. Interest will accrue at 1 'h%per month from the 10th day after completion until paid in full for any unpaid balance not collected upon completion and reasonable attorney fees will be assessed for collection and by agreement of the parties, the venue for judgment shall be Jones County, Iowa, if that is necessary.
