HomeMy WebLinkAboutUniversity of Northern Iowa - MOU - 11/21/2005 Memorandum of Understanding Between the Waterloo Fire Rescue And University of Northern Iowa I. Cooperative Agreement This Memorandum of Understanding (MOU)is made and agreement entered into by and between the Waterloo Fire Rescue, hereinafter referred to as the "Fire Rescue", and the University of Northern Iowa, hereinafter referred to as "UNI." The purpose of this MOU is for the Fire Rescue and UNI to enter into a collaborative partnership in the planning, design and completion of an Evaluation of Fire Pals Education Program ("Study") within Black Hawk County, Iowa, under the terms of this agreement. The Fire Rescue's source of funding for this agreement is the Centers for Disease Control, CFDA# 93.000. The respective responsibilities and obligations for the Fire Rescue and UNI in their mutual desire and intent to exercise the collaborative partnership are as follows: II. Responsibilities A. Responsibilities of the Fire Rescue: 1. Provide adequate general project parameters and guidance relating to the Study's structure and scope, including a mutually agreeable timetable for completion. 2. Utilize, exclusively for the purposes of this partnership, the services of UNI through its Project EXPORT Center on Health Disparities. 3. Provide UNI with adequate background information relating to the successful completion of the Study, including project goals. 4. Secure formal authorization for the Study,under the terms of this MOU, and provide ongoing oversight and coordination of the project, make key Fire Rescue staff available as requested, and facilitate the enlistment of relevant community and school representatives to assist with the successful completion of the Study. 5. This is a fixed-price, performance-based agreement. The Fire Rescue shall pay UNI $9,850.00 in two progress payments upon completion and acceptance of the deliverable items in Article IV. B. Responsibilities of UNI: 1. Assign Annette Vander Werff as Project Director who, along with appropriate staff through its Project EXPORT Center of Excellence on Health Disparities and Global Health Corps, will conduct the Study, including meeting with the Fire Rescue to coordinate activities. 2. Design appropriate survey/interview protocols and instruments to maximize the ability to collect evaluation information from study subjects. 3. Will use appropriate human subjects procedures and documents to solicit parental consent and student subject assent for participation in the Study. 4. Compile, write and produce the Study report to be delivered no later than September 30, 2006. 5. Conduct a cultural competency training session and provide resources as mutually agreed by the partnership. III. Time Line October-November 2005 Develop Study design and protocols Develop instrument for original data collection Conduct cultural competency training for fire fighters November 2005—May 2006 Conduct original data collection June—July 2006 Data analysis August 2006 Write final report IV. Deliverables 1. Instrument for original data collection due November 30, 2005 - $3000. 2. Final report due August 31, 2006 - $6850. V. Publication As an educational institution, UNI may have interest in using findings from the study for educational and professional publication purposes. Publication is understood to include any form of presentation (e.g., paper, report, bulletin, articles) and any format(e.g., written, oral, electronic). If it chooses to pursue publication opportunities, UNI will give written prior notice of such intent to the Fire Rescue. At the Fire Rescue's discretion, a note of disclaimer and/or credit will be included in any such publication. VI. Confidentiality UNI will maintain confidentiality of respondents who are participants in the study. No information about the project, its procedures or results will be disclosed in a way that could lead to the identification of these individuals as participants in the study or to the individual responses provided by respondents. The Fire Rescue also agrees to maintain this same level of confidentiality. The MOU shall be effective October 1, 2005 upon execution of this agreement by both parties and shall continue in effect through August 31, 2006 or until all responsibilities are discharged, whichever is earlier. The deliverable dates may be extended and other modifications made to this agreement only by written agreement of the parties hereto. Date ld 3 Za 5— dward M.
