Change Order No. 5 for the Contract with Woodruff Construction related to the above -referenced project
is enclosed. This change order captures several modifications to the original Contract resulting in an
addition of $55,566.00 to the Contract price. In addition, this change order also results in a change to the
completion date of the project because of impacts to the supply chain from COVID-19 in early 2021,
resulting in a delay to the procurement of equipment and construction materials.
As of Woodruff Construction's most recent pay request for the period ending on November 30, 2021,
this project is 88.1 percent complete with 84 percent of the Contract time elapsed. The total value of
changes to the original Contract price are summarized in the following table.

Description of Change
5a Provide galvanized plate lintels at the four overhead
door openings in Structure 330 in accordance with the
enclosed cost proposal dated July 26, 2021.
5b Provide gate valve in new 4-inch W2 line in
Structure 170 and gate valve in new 4-inch W2 line in
Structure 190 in accordance with the enclosed Cost
Proposal Request (CPR) 013 dated August 17, 2021.
5c Provide bearing plate for the insulated metal panel at the
far southwest end of Structure 330 in accordance with
the enclosed cost proposal dated October 1, 2021.
5d Provide identification for process interconnections
piping and equipment in accordance with the enclosed
CPR 014 dated August 26, 2021.
5e Provide modifications to storm piping near
Structure 350 in accordance with the enclosed cost
proposal dated September 27, 2021.
5f Provide partial demolition of abandoned tank walls
northwest of Structure 190 in accordance with the
enclosed CPR 016 dated September 7, 2021.
5g Amend Contract Substantial Completion date and
Contract Final Completion date for time lost for
construction material procurement as a result of
COVID-19 impacts on manufacturing and supply
