RESOLUTION NO. 2022-392
RESOLUTION APPROVING RENEWAL OF CONTRACT WITH
DELTA DENTAL OF IOWA, IN THE AMOUNT OF $5.06 PER
CONTRACT FOR ADMINISTRATION OF THE EMPLOYEE
DENTAL CARE BENEFIT PLAN FROM JULY 1, 2022 THROUGH
JUNE 30, 2023, AND AUTHORIZING THE HUMAN RESORCE
DIRECTOR TO EXECUTE SAID DOCUMENT.
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF WATERLOO,
IOWA, that the contract dated June 20, 2022, for the renewal of a dental health benefit plan
between Delta Dental of Iowa and the City of Waterloo, Iowa, to administer the employee dental
health benefit plan, including administrative and claims services, at a cost of $5.06 per employee
per month, for the period of July 1, 2022 through June 30, 2023, is hereby approved and the
Human Resource Director is authorized and directed to execute the same on behalf of the City of
Waterloo, Iowa.
PASSED AND ADOPTED this 20th day of June 2022.
