Terracon Consultants, Inc. (Terracon) appreciates the opportunity to submit this proposal for well
plugging services at the above referenced site and submit the associated documentation to the required
regulatory entities. An outline of the project, Terracon's scope of services, including schedule and
compensation are provided in the following sections.
1.0 PROJECT INFORMATION
In correspondence dated September 27, 2022, the Iowa Department of Natural Resources (IDNR)
approved the No Action Required (NAR) classification and indicated that the site is eligible to receive a
No Further Action (NFA) certificate once the necessary documentation for the certificate is provided.
The following must be submitted to the IDNR to issue an NFA certificate:
1. A copy of the most recent deed or contract of sale with an accurate and complete legal
description. NOTE: A legal description obtained from a tax form is not acceptable.
2. Copies of the Abandoned Water Well Plugging Records (DNR Form 542-1226) if they have not
already been submitted. Please remember to include the LUST number on each Well Pluaginq
Record and provide the appropriate signatures on each form.
Once the NFA certificate is issued by the IDNR, the City of Waterloo is responsible for filing the certificate
with the Black Hawk County Recorder and providing the IDNR with a file -stamped copy within 30 days.
The IDNR requires a file -stamped copy for the NFA to be considered effective. This proposal presents
the cost estimate of fees for monitoring well plugging field activities.

2.0 SCOPE OF SERVICES
• Terracon
Terracon has a 100% commitment to the safety of all its employees. As such, and in accordance with
our Incident and Injury Free® safety culture, Terracon will generate a project safety plan for use by
personnel during field services. Prior to commencement of on -site activities, Terracon will hold a meeting
to review health and safety needs for this specific project. At this time, we anticipate performing
fieldwork in a United States Environmental Protection Agency (USEPA) Level D work uniform consisting
of hard hats, safety glasses, protective gloves, and steel -toed boots. It may become necessary to
upgrade this level of protection, at additional cost, during sampling activities in the event that petroleum
or chemical constituents are encountered in soils or groundwater that present an increased risk for
personal exposure.
lase Service'
The scope of this proposal is based upon plugging the reported site monitoring wells. Site records
indicate five wells are associated with the site.
The monitoring wells will be plugged in general accordance with Iowa Administrative Code (IAC) Chapter
39 and IDNR requirements. The services will generally consist of removing the well casing and screen,
filling the remaining hole to the ground surface with hydrated bentonite, removal of the surface
protection (man way and/or ring for flush mount), and replacement of surface to match surrounding
conditions. Alternatively, if the well casing and screen cannot be removed due to damaged casing,
equipment access limitations, etc., services will consist of the following:
■ Remove the surface protection.
■ Remove casing to a depth of four feet below grade at each well.
■ The remaining well annulus will be filled with hydrated bentonite.
■ Backfill the remaining well excavation with soil to an elevation that promotes
drainage away from the plugged well area.
■ Repair the surface to match surrounding existing conditions. For wells in areas of
concrete/asphalt or gravel, a concrete patch will be installed at ground surface or a
layer of gravel will be added as appropriate. For wells in areas of green space, fill
material will be sourced onsite and grass seed will be utilized as needed.
An Abandoned Water Well Plugging Record (DNR Form 542-1226) will be completed for each plugged
well. The forms will be signed by an Iowa -licensed well contractor and require signature from the City
of Waterloo prior to submittal to the IDNR and Black Hawk County Environmental Health Department.
The City will need to provide a legal description obtained from the most recent deed recorded with the
Black Hawk County Recorder. If requested, Terracon will facilitate submittal to the IDNR and Black
Hawk County Environmental Health Department with a request for the NFA certificate. However,
Terracon assumes the certificate will be filed with the Black Hawk County Recorder and a file -stamped
copy provided to the IDNR by the City of Waterloo.

Additional Services Beyond Base Services

Fees for proposed services included in this proposal are listed in Table 1. Please note that the table is
not all-inclusive. Some services will require additional units based on site -specific circumstances.
Should additional services be required beyond those outlined in the Table 1, a supplemental scope and
fee will be developed for client approval.

Services will be initiated upon receipt of the written notice to proceed. We estimate that the project
can be scheduled within 15 business days and field activities completed in one day, weather and site
conditions permitting. In order to comply with the proposed schedule, the following items are needed
to be provided by the client at the time of notification to proceed in order to meet the client's required
project completion date. Please include the following requested items along with the notification to
proceed:
■ Right of entry to conduct the field services.
■ Notification of any restrictions or special requirements (such as safety) regarding
accessing the site.
• Locations of any buried tanks or other subsurface structures, including site service
utilities, known to exist in the area.
• A signed Supplement to Agreement for Services (Agreement) evidencing acceptance
of this scope of services.
Scope and Report Limitation
The findings and conclusions presented in the final report will be based on the site's current utilization and
the information collected as discussed in this proposal. Please note that we do not warrant database or
third -party information, or regulatory agency information used in the compilation of reports.
The limitations herein must be considered when the user of this report formulates opinions as to risks
associated with the site. No warranties, express or implied, are intended or made.
Site Access and Safety
Client shall secure all necessary site related approvals, permits, licenses, and consents necessary to
commence and complete the Services and will execute any necessary site access agreement. Consultant
will be responsible for supervision and site safety measures for its own employees but shall not be
responsible for the supervision or health and safety precautions for any third parties, including Client's
contractors, subcontractors, or other parties present at the site. In addition, Consultant retains the right
to stop work without penalty at any time Consultant believes it is in the best interests of Consultant's
employees or subcontractors to do so in order to reduce the risk of exposure to the coronavirus. Client
agrees it will respond quickly to all requests for information made by Consultant related to Consultant's
pre -task planning and risk assessment processes. Client acknowledges its responsibility for notifying
Consultant of any circumstances that present a risk of exposure to the coronavirus or individuals who
have tested positive for COVID-19 or are self -quarantining due to exhibiting symptoms associated with
the coronavirus.

3.0 COMPENSATION
• Terracon
The Scope of Services outlined in this proposal will be performed as a unit price not to exceed $2,725
in accordance with the attached budget estimate presented in Table 1. If, as a result of these services,
additional work is required outside the scope of this proposal, you will be contacted and proposed fees
for additional work will be provided. Client authorization will be obtained prior to commencement of
additional work outside the scope of this proposal. Additional fees will be incurred if City of Waterloo
requests Terracon assist with filing the NFA certificate with the Black Hawk County Recorder and
providing a file -stamped copy to the IDNR. These additional fees would be estimated at $500 depending
on the Black Hawk County filing fee and submittal requirements.
4.0 AUTHORIZATION
If this proposal meets with your approval, work may be initiated by returning an original copy of the
attached Supplement to Agreement for Services to our Cedar Falls, Iowa office. Project initiation may
be expedited by sending a copy of the Task Order via e-mail to Dave.Cleary@terracon.com or facsimile
at (319) 277-4320.

SCOPE OF SERVICES AND FEES
This SUPPLEMENT to AGREEMENT FOR SERVICES to the original Agreement for Services (original Agreement dated 02/01/2021, Agreement
reference number P13207110) is between City of Waterloo IA ("Client") and Terracon Consultants, Inc. ("Consultant") for additional or changed Services to
be provided by Consultant for Client on the Project, as described in the Agreement for Services. This Supplement is incorporated into and part of the
Agreement for Services.
Scope of Services. The scope of the additional or changed Services are described in the Scope of Services section of the Consultant's
Supplemental Proposal, unless Services are otherwise described below or in Exhibit B to this Supplement (which section or exhibit are incorporated
into the Supplement).

Compensation. Client shall pay compensation for the additional or changed Services performed at the fees stated in the Supplemental Proposal
