RESOLUTION APPROVING CLAIMS SERVICE CONTRACT
WITH ALTERNATIVE SERVICE CONCEPTS, LLC OF
NASHVILLE, TENNESSEE, AND DIRECTING EXECUTION
OF SAID CONTRACT BY MAYOR.
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the Claims Service Contract dated December 13, 2010, at a
cost not to exceed $25,000.00, plus bill review, to provide
administrative and claims services for self-funded work comp
insurance, by and between Alternative Service Concepts, LLC of
Nashville, Tennessee and the City of Waterloo, Iowa, be and the
same is hereby approved, and the Mayor and City Clerk authorized
to execute the same in behalf of the City of Waterloo, Iowa.
PASSED AND ADOPTED this 13th day of December, 2010.
