Reason for Change order
5 Subgrade preparation was not necessary and removed from the project due to sandy conditions.
6-7 Change of product which still meets IDOT gradation.
16-19 Switching Concrete Aprons to Steel Aprons.
22 Subdrain was removed for upper parking lot and basketball court areas.
69 Change in dewatering device style.
130 Includes the removal of pumps and associated piping from the splash pad surge tank,
removal of associated conduit, wiring, disconnect, breaker, and controls for SP1 and SP2,
and a new hub drain with removal drain plug as shown on attached Sheet M1.01.
146 Engineering fabric was added as a replacement of subgrade preparation. Quantity was added for basketball court paving.
147 Slide cost share with City.

Dear Ms. Sweeney,
Per direction from email received on 8/18/23, we are proposing the following for removing pumps SP1 and SP2 from
project:
• DEDUCT for removing pumps and associated piping from the splash pad surge tank = ($18,890.00)
• DEDUCT for removing associated conduit, wiring, disconnect, breaker and controls for SP1 & SP2 = ($1,938.00)
• ADD for new hub drain with removal drain plug as shown on the new plans = $315.00
Our total price to perform the subject changed work will be a credit of $20,513.00.
When this potential change order is signed by the Owner and a copy is returned to the contractor, approval of
change(s) is effective IMMEDIATELY and we will proceed with the change(s) described above. Approved changes
