TO ALL BIDDERS ON THE ABOVE PROJECT:
All Bidders submitting a bid on the above-referenced contract shall carefully read this Addendum and give it
consideration in the preparation of their bid.
The following are revisions to the Specifications:
a. In the Form of Bid section,the quantity for"Removal and Replacement of Light Fixtures"has been
revised to 29 to include 1 spare fixture(see attachment)
b. In the Items of Potential MBENVBE Participation section, the quantity for "Removal and
Replacement of Light Fixtures"has been revised to 29(see attachment).
II. The following are revisions to the Plans:
a. On Sheet V.2,the quantity for Item 20,Remove and Replace Light Fixtures, has been revised to 29
to include 1 spare fixture(see attachment).
b. On Sheet V.20, Note 4, an additional light fixture model has been pre-approved. It is McGraw-
Edison(Cooper Lighting)Cat#TT-B6-LED-E1-WQ-BK(see attachment).

IV. Any revisions to any of the Contract Documents made by this Addendum shall be considered as the same
revision to any and all related areas of the Contract Documents not specifically called out in this Addendum.
V. The Bidder shall acknowledge receipt of this Addendum by inserting the date and number in the spaces
provided in the revised FORM OF BID,which is attached and made a part of this Addendum No. 1.

2. It is understood that the quantities set forth are approximate only and subject to variation and that the
unit bid price for the work done shall govern in the actual payment to Contractor.
3. In submitting this bid, the bidder understands that the right is reserved by the City of Waterloo, Iowa,
to reject any or all bids. If written notice of the acceptance of this bid is mailed, telegraphed, or
delivered to the undersigned within thirty(30)days after the opening thereof, or at any time thereafter
before this bid is withdrawn, the undersigned agrees to execute and deliver an agreement in the
prescribed form and furnish the required bond and certificate of the insurance within ten (10) days
after the agreement is presented to him for signature, and may start work after"Notice to Proceed" is
issued and no later June 13, 2016.

8. The Prime Contractor and Subcontractor(s), which have performed an aggregate of $10,000.00 in
work for the City in the current calendar year, are prepared to submit an AAP or Update and an
EOC,within ten (10)days of notification that the bid submitted is lowest and acceptable.
9. The bidder has received the following Addendum or Addenda:

10. The bidder shall list the MBE/WBE subcontractor(s), amount of subcontracts and bid items on the
City of Waterloo Minority and/or Women Business Pre-bid Contact Information Form submitted with
this Form of Bid or Proposal. The apparent low Bidder shall submit a list of all other Subcontractor(s)
to be used on this Project to the City of Waterloo by 5:00 p.m. the business day following the day
Bids on this Project are due along with the Non-Collusion Affidavits of All Subcontractor(s).
The Contractor shall submit information on subcontractors on"SUBCONTRACTOR REQUEST AND
APPROVAL" Form to be provided by City prior to approval of contract.
The subcontractors listed on this proposal and/or submitted to the Contract Compliance Officer
cannot be changed except for the following reasons:

NOTICE TO PROSPECTIVE BIDDERS:
The following are items from the FY 2016 4th Street Bridge Canopy Repairs, City Contract No. 910,
that have a potential for MBE/WBE Participation. This listing, however, is not intended to be all
encompassing; to preclude MBE/WBE Contractors from bidding on any items of the Proposal as a
Subcontractor or the entire Proposal as a Prime Contractor.

MBE/WBE GOALS FOR THIS PROJECT
IMPORTANT: Prime Contract Bidders should be aware that ten percent (10%) MBE goal and a two
percent(2%)WBE goal is attached to this project. "Certified Return Receipt" letters are no longer needed
with the bid showing proof of contacting MBE or WBE Subcontractors, but the Pre-Bid Contact Information
Form must be filled out completely.
