North American Procurement Council PO Box 40445 Grand Junction, CO 81504 TEL 302-450-1923 FAX 302-450-1925 www.NAPC.pro CITY OF WATERLOO, IOWA COMMUNITY DEVELOPMENT BLOCK GRANT PROGRAM CDBG - FUNDED PROJECT SPECIFICATIONS FOR DEMOLITION AND SITE CLEARANCE CONTRACT NO: 181-CDBG-DEMO-01-11-16 INVITATION FOR BIDS The City of Waterloo, Iowa, acting through the Community Development Board will receive bids for demolition of buildings and structures and site clearance for ten (10) structures known as Group 181-CDBG-DEM)-01-11-16, until 3:00 P.M. on the 7th day of January, 2016, in the office of the City Clerk of Waterloo, Iowa. The bids will be publicly opened and read aloud at 4:00 p.m. on the same date in City Hall in the City of Waterloo. Contract Documents, including Drawings and Technical Specifications, are on file at the Community Development Board, Carnegie Annex, Suite 202, 620 Mulberry Street, Waterloo, Iowa 50703. Copies of the Contract Documents may be obtained by depositing $15.00 (in cash or by check payable to the Community Development Board) with the Community Development Director of the Community Development Board for each set of documents so obtained, and your submission of a statement of the Bidder's Qualifications and Financial Condition if requested. DEPOSITS ARE REFUNDABLE if documents are returned within 10 days of the bid opening. CITY OF WATERLOO, IOWA Date: December 18, 2015 Suzy Schares, City Clerk SPECIAL NOTICE TO ALL BIDDERS for Demolition and Site Clearance on City of Waterloo Community Development Block Grant Program CDBG Demolition Project. Please read "INSTRUCTIONS TO BIDDERS" and all Contract Documents carefully and see that all requirements for filing your bid are met. Checking your bid, before filing, against the following list will assist in preventing minor errors or omissions which could result in disqualification of your bid because of technicalities. 1. Bid must be SIGNED, and bear the official title of the person signing. 2. Bid must be submitted SEALED in an envelope addressed to: City of Waterloo CLERK'S OFFICE 715 Mulberry Street Waterloo, Iowa 50703 and must be marked in the lower left-hand corner as follows: BID FOR DEMOLITION AND SITE CLEARANCE COMMUNITY DEVELOPMENT BLOCK GRANT PROGRAM PROJECT NO. 1 8 1-CDBG-DEMO-01-1 1-1 6 TO BE OPENED: JANUARY 7,2016 AT 4:00 P.M. 3. Sufficient time should be allowed for mailed bids to be delivered by normal Postal operation. Late bids will not be considered. Bids must be received by 3:00 P.M., JANUARY 7,2016 in the CITY OF WATERLOO CLERK'S OFFICE. Bid will be awarded JANUARY 11,2016 during regular business of the City Council. 4. ALL BLANKS in the Bid Form must be filled in. 5. NO ERRORS IN ITEMIZED UNIT PRICE SCHEDULE. Itemized Unit Price Schedule must be totaled and the total must agree with the price filled in on BF-1. All bids must include a bid on all parcels and structures. In this project all parcels constitute one contract which shall be let to one bidder. 6. QUALIFIED BIDS NOT ACCEPTED: Bid must not be qualified in any way or contain any reservations not made optional in the Bid Form provided to bidders. 7. YOUR QUOTE should include removal of trees identified along the property line and stumps for all addresses,where appropriate and applicable. THIS SPECIAL NOTICE IS ISSUED AS A REMINDER AGAINST COMMON IRREGULARITIES IN BIDS, AND IS NOT A CONTRACT DOCUMENT. INSTRUCTIONS TO BIDDERS 1. USE OF SEPARATE BID FORMS These Contract Documents include a complete set of Bidding and Contract Forms which are for the convenience of bidders and are not to be detached from the Contract Documents or filled out or executed. Separate copies of Bid Forms and Contract Documents are included with the document. Each bidder should understand that the Community Development Board reserves the right to reject any and all bids. 2. INTERPRETATION OR ADDENDA No oral interpretation will be made to any bidder as to the meaning of the Contract Documents or any part thereof. Every request for such an interpretation shall be made in writing to: Rudy D. Jones, Director Community Development Board 620 Mulberry Street, Carnegie Annex Waterloo, Iowa 50703-5713 Any inquiry received seven or more days prior to the date fixed for the opening of bids will be given consideration. Every interpretation made to a bidder will be in the form of an Addendum to the Contract Documents and when issued will be on file in the office of the Community Development Board at least five days before bids are opened. In addition, all Addenda will be mailed to each person holding Contract Documents, but it shall be the bidder's responsibility to make inquiry as to the Addenda issued. All such Addenda shall become part of the Contract Documents and all bidders shall be bound by such Addenda,whether or not received by the bidders. 3. INSPECTION OF SITE Each bidder shall visit the site of the proposed work and fully acquaint her/himself with the existing conditions there relating to the Demolition and/or Site Clearance and should inform her/himself as to the facilities involved, the difficulties and the restrictions attending the performance of the Contract. The bidder shall thoroughly examine and familiarize her/himself with the Drawings, Technical Specifications, and all other Contract Documents. The contractor by the execution of the Contract shall in no way be relieved of any obligation under it due to his/her failure to receive or examine any form or legal instrument or to visit the site and acquaint her/himself with the conditions there existing and the City of Waterloo will be justified in rejecting any claim based on facts regarding which he/she should have been on notice as a result thereof 4. RELEASE OF BUILDINGS: SEQUENCE OF WORK Bidders are referred to the Pre-Construction Meeting for information regarding the manner in which the existing buildings will be released and the site made available for Demolition purposes, and the sequence in which the Demolition work will be preformed. 5. ALTERNATIVE BIDS Alternative bids will be rejected unless specifically requested. 6. BIDS A. All Bids must be submitted on forms supplied by the City of Waterloo and shall be subject to all requirements of the Contract Documents including the Drawings, and these INSTRUCTIONS TO BIDDERS. All bids must be regular in every respect and no interlineations, excisions or special conditions shall be made or included in the Bid Form by the bidder. B. Bid Documents including the bid, the Non-Collusions Affidavit,Non-Segregated Facilities, and the Statement of Bidder's Qualifications (if requested) shall be enclosed in a sealed envelope clearly labeled with the words ("Demolition and Site Clearance Bid Documents"), project number, name of bidder, and date and time of bid opening in order to guard against premature opening of the bid. C. The City of Waterloo may consider as irregular any bid on which there is an alteration of or departure from the Bid Form hereto attached and at its option may reject the same. D. If the Contract is awarded, it will be awarded by the City of Waterloo to a responsible bidder on the basis of the bid most favorable to the City. The contract will require the completion of work according to the Contract Documents and bonding requirements. E. Each bidder shall include in his/her bid, in the appropriate spaces therefore, the estimated cost of performing the work of Demolition and Site Clearance including all items of overhead, and without credit for salvaged materials. F. Each bidder shall include in his/her bid the following information: Principals Firm Names Name Social Security Numbers Federal Tax/I.D. Number Home Addresses (including City, City, State and Zip Code State and Zip Code.) 7. COLLUSIVE AGREEMENTS A. Each bidder submitting a bid to the City of Waterloo for any portion of the work contemplated by the documents on which bidding is based shall execute, and attach thereto, an affidavit substantially in the form herein provided, to the effect that he has not colluded with any other person, firm, or corporation in regard to any bid submitted. B. Before executing any subcontract, the successful bidder shall submit the name of any for prior approval and an affidavit on the form provided. 8. STATEMENT OF BIDDER'S QUALIFICATIONS A. Each bidder shall upon request of the City of Waterloo submit on the form furnished for that purpose, a copy of which is included in the Contract Documents, a Statement of the Bidder's Qualifications, his demolition experience, and his organization and equipment available for the work contemplated; and when specifically requested by the City, a detailed financial statement. The City of Waterloo shall have the right to take such steps as it deems necessary to determine the ability of the bidder to perform his obligations under the Contract, and the bidder shall furnish the City all such information and data for this purpose as it may request. Each bidder shall provide a current copy of a Certificate of Insurance with the Community Development Department prior to submission of bid. THE RIGHT IS RESERVED TO REJECT ANY BID WHERE AN INVESTIGATION OF THE AVAILABLE EVIDENCE OR INFORMATION DOES NOT SATISFY THE CITY THAT THE BIDDER IS QUALIFIED TO CARRY OUT PROPERLY THE TERMS OF THE CONTRACT. 9. CORRECTIONS Erasures or other changes in the bid must be explained or noted over the signature of the bidder. 10. TIME FOR RECEIVING BIDS A. Bids received prior to the time of opening will be securely kept unopened. The officer whose duty it is to open them will decide when the specified time has arrived, and no bid received thereafter will be considered; except that when a bid arrives by mail after the time fixed for opening, but before the reading of other bids is completed, and it is shown to the satisfaction of the City that the non-arrival on time was due solely to delay in the mails for which the bidder was not responsible, such bid will be received and considered. Among the factors to be considered are: the post-mark date of the bid, the normal delivery time for the distance in question, based upon local postal deliveries. B. Bidders are cautioned that, while telegraphic modifications of bids may be received as provided above, such modifications, if not explicit and if in any sense subject to misinterpretation, shall make the bid so modified or amended, subject to rejection. 11. OPENING OF BIDS At the time and place fixed for the opening of bids,the City will cause to be opened and publicly read aloud every bid received within the time set for receiving bids, irrespective of any irregularities therein. Bidders and other persons properly interested may be present, in person or by representative. 12. WITHDRAWL OF BIDS Bids may be withdrawn on written or telegraphic request dispatched by the bidder in the time for delivery in the normal course of business prior to the time fixed for opening; provided, that written confirmation or any telegraphic withdrawal over the signature of the bidder is placed in the mail and postmarked prior to the time set for the opening. 13. AWARD OF CONTRACT: REJECTION OF BIDS A. The entire Contract will be awarded to the lowest qualified responsible bidder complying with the conditions of the INVITATION FOR BIDS. The City, however, reserves the right to reject any and all bids and to waive any informality in bids received whenever such rejection or waiver is in its interest. The bidder to whom the award is made will be notified at the earliest possible date. B. The City of Waterloo reserves the right to consider as unqualified to perform the Contract any bidder who does not habitually perform with his/her own forces the major portions of the work involved in the Demolition and Site Clearance. 14. EXECUTION OF AGREEMENT: BID BOND A. The bid must be accompanied by a bid guaranty which shall not be less than five percent (5%) of the total estimated cost of Demolition and Site Clearance including all items of overhead and without credit for salvaged materials. At the option of the bidder, the guaranty may be certified check, bank draft, negotiable U.S. Government bonds (at par value), or a Bid Bond in the form attached. The Bid Bond shall be secured by a guaranty or surety company listed in the latest issue of the U.S. Treasury, Form 570. The amount of such Bid Bond shall be within the maximum amount specified for such company in said Form 570. No bid will be considered unless it is accompanied by the required guaranty. Certified check or bank draft must be payable to the order of the City of Waterloo, Iowa. Cash deposits will not be accepted. The bid guaranty shall insure the execution of the Agreement and the furnishing of the surety bond or bonds by the successful bidder, all as required by the Contract Documents. SCHEDULE OF DRAWINGS DEMOLITION AND SITE CLEARANCE GROUP 181-CDBG-DEMO-01-11-16 201. SCOPE OF DRAWINGS The contract drawings listed in Section 203 below, indicate primarily only the location, size and general character of the principal buildings and features within the parcels which have been acquired by the City of Waterloo, and which are included in work under this contract. Numerous other items of work of a relatively minor nature will be found in the Technical Specifications, and it is solely the contractor's responsibility to determine the extent of all work required by the specifications, but not shown on the drawings, all of which he will be required to perform. 202. INTERPRETATION OF DRAWINGS If dimensions are shown for buildings they are nominal and approximate, and the contractor shall verify al].dimensions and materials to his own satisfaction before submitting his bid on the work. 203. SCHEDULE OF DRAWINGS (Exhibit C) The following map indicates the location of the single structures scheduled for demolition.
